Twin Woods Junior Golf League
Summary
The 2016 Twin Woods Junior Golf League is for boys and girls, 10 to 17 years old.
The league will operate every Thursday morning for eight weeks, starting June 16 and ending with a tournament
and awards lunch on August 11.
The registration fee is $110 and the green fee for league members is $14 each week. Checks should be made
payable to “Twin Woods Junior Golf League.”
Lessons are included in the league registration fee and will be given before the golfers play golf. They will be
taught by our golf pro, a certified golf instructor.
League members will be issued a “League Card” and will be able to play golf other weekdays at the league rate
of $14 for nine holes or $19 for 18 holes. Pull carts are available at the reduced fee of $1 per 9-hole round.
We plan to assemble four flights of golfers according to their abilities and based on their handicaps.
To qualify for the tournament, golfers must have recorded at least five scores during the seven prior weeks of
league play. Following the tournament, golfers will be invited to an awards presentation and a light lunch.
Registration
These four things are required for a complete registration:
1. Registration form.
2. Registration fee: $110 cash or check payable to “Twin Woods Junior Golf League.”
3. At least one scorecard with your scores from any course, so we can place you in the proper flight.
4. A parent/guardian release form.

Tee Times and Lessons
We plan to have four starting times for lessons, in order to allow golfers with similar scores to learn and play
together. Golfers will be notified by E-mail of their starting times before league play begins each week and they
should arrive at the golf course 15 minutes before their lessons start. Tee times to play golf will be ½ hour
following the first starting time at 10 a.m.

Please include with this application your completed Waiver and Release Form and at least one scorecard from
golf games played in 2015 or 2016.
I understand that my signed Participant’s Waiver and Release form is part of my registration. I also
understand that primary insurance coverage is my individual or family plan. I have no objections to Twin
Woods Golf Course using a photograph of my child in promotional materials. I understand that this

2016 TWIN WOODS JUNIOR GOLF LEAGUE
PARTICIPANT’S WAIVER AND RELEASE
The Twin Woods Golf Course will sponsor the Twin Woods Junior Golf League, hereafter called The
League, at the golf course at 2924 E. Orvilla Road, Hatfield, PA, during the summer of 2016.
I certify for my minor child that the Twin Woods Prep Golf League participant is in good health and
capable of participating in The League. I, therefore, desire that my minor child participate in The
League.
In consideration of the foregoing, for my minor child’s participation in The League and for other
valuable consideration for which receipt of this waiver and release is hereby acknowledged, I do,
hereby, agree for myself, my heirs, executors, administrators, and assigns, forever remise, release
and discharge Twin Woods Golf Course and their directors, officers, members, agents, and any other
representative related to The League, and their heirs, executors, administrators, and assigns from
any and all actions, causes of action, suits, debts, accounts, controversies, damages, claims and
demands, whatsoever, which I or my legal representative may have or acquire against The League
and Twin Woods Golf Course and their directors, officers, members, agents and any other
representative related to The League, by reason of any loss resulting from personal injury or property
damage, which may occur during or by reason of my minor child’s participation in The League.
I agree that Twin Woods Golf Course shall have the right, at its discretion, to enforce rules of conduct
and/or terminate my minor child’s participation for failure to act in conformance thereof, or for his or
her actions or conduct if detrimental to or incompatible with the welfare, comfort, harmony, course
rules or interest of the group or The League as a whole. Golf course etiquette is taught and is strived
for by all.
I hereby grant Woods Golf Course and their directors, officers, members agents and other
representatives full authority to take whatever action, in their discretion, is determined to be
necessary regarding my minor child’s health, safety and welfare, and I fully release Twin Woods Golf
Course and their directors, officers, members, agents and other representatives from any liability for
such actions, as set forth herein.
