Consent and Release:
This document is to certify that I in consideration of the benefits to my child / children_________________________________
____________________________________________________________________ and other children to be gained through
their Participation in the athletic activities sponsored by the Morrisville Little Bulldogs Youth Football and Cheerleading
Organization, it's organizers, sponsors, officers, managers, coaches, and members of said organization, do hereby release and
agree to save harmless the said Morrisville Little Bulldogs Youth Football and Cheerleading Organization and the organizers,
sponsors, officers, managers, coaches, and members of any other school or organization participating in such athletic activities
with the Morrisville Little Bulldogs Youth Football and Cheerleading Organization, from any and all claims or actions whatsoever
based upon the transportation of the participants, or the playing, equipment, or operation and / or management and coaching
said participants team or organization in the activity sponsored by the Morrisville Little Bulldogs Youth Football and Cheerleading
Organization.
I, the undersigned parent or legal guardian, do hereby grant permission to any licensed physician to perform or provide
necessary emergency medical care or aid to my child named above in connection with the playing of Football or Cheerleading.
As the Parent / Guardian of the above named child / children, I accept full responsibility for all of the Football / Cheerleading
equipment / uniforms issued to the above named participant by the Morrisville Little Bulldogs Youth Football And Cheerleading
Organization. This responsibility includes cleaning and maintaining the uniforms to keep them in good condition. Equipment
including game and practice uniforms are to be worn for organized Morrisville Little Bulldogs practices, games, and events only.
All issued equipment must be returned either at the time the participant resigns, is removed from the team, or at the end of the
season at a time and place to be determined by the organization. I agree to pay for any and all equipment not returned in an
acceptable condition at the actual replacement cost. "Acceptable condition" will be determined by the Morrisville Little Bulldogs
Board and its decision will be final and binding. Full equipment / uniform replacement cost is $400 for Football participants, and
$125 for Cheerleading participants.
I also agree to pay any legal fees incurred by the Morrisville Little Bulldogs Youth Football and Cheerleading Organization if legal
action is required to fulfill this contract.
The cost of maintaining damaged or broken equipment during the season will be incurred by the Morrisville Little Bulldogs as
long as the damage or equipment brake was due to normal use. "Normal use" will be determined by the Morrisville little
Bulldogs Youth Football and Cheerleading Organization Board and its decision will be final and binding.
I agree to return upon request the uniform and all other equipment issued to my child named above in good condition or be
financially responsible to replace it. I agree to abide by all rules and regulations set up by Morrisville Little Bulldogs Youth
Football and Cheerleading Organization and it sanctioning bodies (AYF & Little Guys Conferences) governing this sport.
Also, please note that the American Youth Football and Little Guys Conference rules dictate that all Football players are required
to wear and athletic cup. If the player is not wearing this during practice and / or games the participant will be ruled ineligible to
participate and removed from the field.
I hereby consent to and authorize the use and reproduction by the Morrisville Little Bulldogs Youth Football and Cheerleading
Organization or anyone authorized by the Morrisville Little Bulldogs Youth Football and Cheerleading Organization, of any and all
photographs, video, or other image that have been taken or will be taken during the course of this football / cheerleading season
of my child for any purpose without compensation to me. All negatives and Positives, together with the prints, are owned by the
Morrisville Little Bulldogs Youth Football and Cheerleading Organization. The Organization reserves the right to use these
images in any of its print or electronic publications and will not be sold to any outside sources for use.
No Refunds will be issued for any reason after August 15 for Football players and August 31 for cheerleaders.
