MASE Wellness Screenings 2017 Instructions for Scheduling and for Completing the Patient Questionnaire (HRA) Online Getting Started 1. Access the internet and enter the web address: https://www.wecaretlc.com/clinic Note: If the user has Pop-up Blocker on, the pop-ups such as the Patient Questionnaire reminder will be blocked. The pop-up blocker needs to be turned off for this site. 2. Fill in the following items: **(Every participant has their own unique login, even children)** Group ID: Each MASE Health & Wellness Center (Clinic) will have their own Group ID sign in: MASE Plymouth - Use: MASEP MASE Monticello - Use: MASEM New Prairie – Use: MASENP Eastern Pulaski – Use: MASEEP South Central – Use: MASESC South Newton – Use: MASESN Delphi --------- Use: MASECC User ID: For example: Insured ID # = Found on your Insurance ID Card, without the YRP Sex=M or F Birth year = 1977 Password: Last name in all CAPITAL letters; if you have previously accessed your profile and have changed your password, enter the password that you chose at that time. If this is your first visit to the website, you will be prompted to change the temporary password assigned upon initial login to a new password. Once your new password is entered click submit and you will then be directed to the Home Page. ***If you are new to this website, you will be asked to choose which location you will be utilizing. Choose the MASE Insurance Trust (Non-Clinic Location). If are already a Monticello or Plymouth location user you will be navigated directly to the Home Page. Note: If you cannot get into the Employee Health Clinic site using the above instructions, please call 1-800-941-0644 and press option 2. MASE Plymouth Clinics at 1-877-289-3011, option 2 MASE Monticello 1-844-223-2964 or 574-297-5527 Home Page Completing the On-line Health Assessment (Patient Questionnaire, also known as the Health Risk Assessment) 1. On the main Home page, click on the Patient Questionnaire link on the left side of the page and then click on the Health Assessment Event 2017 button. 2. Answer the questions. Note: additional questions may appear depending on your responses. 3. Don’t forget to SAVE your responses at the very bottom of the page. 4. If the lab values have been completed and downloaded into your profile, you will be able to see your Patient Questionnaire Report (HRA) immediately. If the labs are not available, you will get a message to check back in 3 to 5 days after lab work has been done to view your full Health Evaluation. Scheduling an Appointment 1. On the main Home page, click on the SCHEDULE link. 2. Verify your Personal Information on the screen; Click Save at the bottom of the screen. 3. Next - Location, choose ‘MASE Insurance Trust-Non Clinic Location’ from the dropdown. 4. Next, select ‘Fasting Lab’ for the Visit Type from the dropdown. Select “Next” 5. Choose a day and time that works for you. Select “Next” 6. The confirmation page will appear with your scheduled appointment date and time; on this screen please read the special instructions, “Nothing to eat or drink 10 hours prior to testing. Water and black coffee is acceptable and encouraged.” In the “Additional Information” area, please note “HRA Labs”. Confirm and then click FINISH. 7. To cancel you must call the clinic. To reschedule your visit, go to the Home Page and your scheduled time will appear in the upper right-hand corner. Click on the appointment and then reschedule for the time desired following these same directions. ** Remember, you can always contact the Monticello or Plymouth clinics for assistance or to schedule your Wellness Screening visit. You must, however, complete the Patient Questionnaire (Health Risk Assessment) online. If you do not have access to a computer, please let us know.
