Proposal to provide meeting evaluation service

The event representative above wishes to contract with AAPM to host a meeting evaluation
service for the above named event for a fee of $500. AAPM will provide a link to a simple
evaluation system that requires the user to enter first name, last name and email address, and
fill out a CAMPEP approved form for each session they wish to evaluate.
The event representative agrees to allow AAPM access to the event’s CAMPEP application in
order to gather session information for the service. The evaluation service will close 14 days after
the event date. After the system is closed, it will not be re-opened. Within 10 business days of
survey close, AAPM will provide the event representative with a spreadsheet ready for upload
to the CAMPEP system.
