This application must be used by all individuals applying to be a coach with a NDUFC team. This includes coach & assistant coach
positions. A new application is required each year. To be rostered as a "Coach" of one of our teams you must have attained
(or plan to attain) your OSA Senior Level Coaching certification. Interviews may be required for new and competing applicants.
All team officials must be screened each year. This may or may not require a Police Check. All Applicants, by submitting this form,
agree to abide by the constitution, by-laws and policy & procedures of the NDUFC & all its governing bodies (DRSA, OSA & CSA).
