your Personal Health Assessment (PHA).

completing the PHA in 2017.
What is the PHA?
ll A
confidential online survey to evaluate your
overall well-being.
ll It
identifies behaviors and characteristics that
impact your health.
ll It
helps us give you personalized advice on
the best ways for you to get healthier.
ll Remember,
you and your spouse (if covered)
must complete the PHA in the Member
Wellness Portal before any wellness activities
count for points.
ll If
you log any activities prior to completing
your PHA, you’ll have to re-enter them after
you take it for the points to count.
Register for BlueAccess
ll Visit
bcbst.com/Nissan-Wellness.
ll Choose
the Register Now option under Log In/
Register to BlueAccess.
ll Select
ll You’ll

Complete Your PHA

the AlwaysOn® app:
¡¡ Download the AlwaysOn app from the App Store® or Google Play®.
your computer:
¡¡ Visit bcbst.com/Nissan-Wellness and
log in to BlueAccess.
¡¡ Open the app and tap Enter Keycode
at the bottom of the screen.
¡¡ Click on My Health & Wellness at the
top of the page.
¡¡ Enter the keycode: Nissan
¡¡ Enter your first name, last name,
date of birth and zip code.
¡¡ Select Member Wellness Portal.
¡¡ Choose Get Started.
¡¡ Create a username and password, enter your phone number and accept the terms and conditions.
¡¡ You’ll be taken to your personalized wellness page and prompted to take
your PHA.
¡¡ Create a device PIN.
¡¡ Click Get Started Now! and complete your PHA.
¡¡ Open the app, select Health Assessment and take your PHA.
