Procedure
1. BCSPL team wishing to obtain permit for a district registered youth player completes and signs Section 1 and 2 in its entirety
and forwards the application to the player’s current youth team official. An email address must be provided so that the approved
permit can be returned to the BCSPL team official.
2. The Releasing Youth Team official completes Section 3 and forwards the permit application to their Youth District Official which
is the Releasing Youth District for approval. The player’s district ID card is required with this form at the match noted.
3. The Youth District Official completes Section 4 and the permit application is faxed to 1-866-203-0044 or e-mailed to:
[email protected] at least 24 hours prior to the match.
