WINNERS AGREEMENT FORM
Players Player of the Year
I sign this document to confirm that I understand to return this award in the same
condition as I have received it. Any damages to this award shall be my liability and I will pay
the costs incurred by Salvation Army Football Club for any repairs required for damage
caused. Should I lose the award I shall pay for a replacement as well as any retrospective
engraving as considered by Salvation Army Football Club.
In addition if I fail to re-register with the club or move from the area I will return the award
with immediate effect.
