Kappa Delta Campus Golf
Benefiting Prevent Child Abuse America and Avalon
Campus Golf is Kappa Delta’s annual competition that combines a day of fun, nine-hole courses that
will be set up on different landmarks around campus. Each team will be provided with tennis balls and golf
clubs to make their way through the course with the least amount of strokes. A Kappa Delta will be assigned to
caddy for each team. Any William and Mary student or faculty member is invited to participate!
When: Saturday, February 27, 2010!
Team captains will be
informed of their tee time through email by Friday Feb. 26. It is the team
captain’s responsibility to notify the rest of the team of its tee time. If
teams show up at incorrect tee times, they will not be allowed to tee-off.
Registration: Register your team by going to the SC during lunch or
dinner hours from Feb. 18th- Feb. 19th and Feb. 22nd – Feb. 24th, or by
giving your form to any Kappa Delta sister by Wednesday, FEBRUARY
24th. No late forms will be accepted. If you register during the first
week, your team will receive your t-shirts on the day of Campus Golf.

$10 per player, 4-5 players per team, Shamrock T-shirt included!
Please make checks payable to Alpha Pi of Kappa Delta. Cash is also accepted.
Campus Golf is an alcohol free event. Consumption of alcohol before or during Campus Golf is against
Kappa Delta policy and any person showing signs of inappropriate conduct due to consumption of alcoholic
beverages may be asked to leave without refund.

Informed Consent Form
I, the undersigned, hereby agree to the following:
1. I am voluntarily participating in Kappa Delta Campus Golf at Kappa Delta Sorority, Alpha Pi chapter at the
College of William and Mary (hereinafter referred to as “Kappa Delta”). I recognize that participation in this
event requires physical exertion and I am fully aware of the risks and hazards involved and the rules of the
event.
2. I represent and warrant that to the best of my knowledge I have no medical condition that would prevent my
safe participation in the event. I further represent and warrant that I will participate in the event, follow all
rules and regulations, act in a responsible manner, and not bring any food, drink or other substance to the
event that might impair my ability to participate. I understand that I might be asked to forego participation if
it is believed that I am unfit to participate.
3. In consideration of Kappa Delta’s permitting me to participate in Campus Golf, I hereby agree to the
following conditions:
a. I assume full responsibility for any risks, injuries, or damages, known or unknown, which I might
incur as a result of the participation in Kappa Delta Campus Golf.
b. I knowingly, voluntarily, and expressly waive any claim I may have against Kappa Delta Sorority,
Inc., Alpha Pi Chapter of Kappa Delta Sorority, Inc., Alpha Pi parents club, Alpha Pi house
corporation or Alpha Pi chapter advisory board and their officers, agents, and employees, for any
injury or damages sustained whatsoever as a result of participation in Kappa Delta’s Event.
c. I agree to abide by all other terms and conditions required for participation in KD Campus Golf as
they may from time to time be posted, amended or augmented.
I have read the above liability waiver and informed consent form and fully understand its contents. I voluntarily
agree to the terms and conditions stated above.
