Please complete this section and forward it to your reviewer 5 days before the review meeting. PART 1 – REVIEW AND CONTRIBUTION Provide a summary of your main activities during the last 12 months Evaluate your progress against previously agreed objectives. Evaluate your successes and what has gone well during the period. Also note if any factors hindered your progress during the year. Outline the development activities you have undertaken during the last review period and the impact they have had on your performance. Also note any issues that may have arisen regarding your workload or working relationships that have had a negative effect on your ability to undertake your duties effectively. PART 2 – LOOKING FORWARD What do you hope to achieve in the next 12 months. (Please consider what support you may need to achieve those aims, what resources you may need and how you will measure your progress towards achieving your objectives. Also consider how they will support the overall objectives of the University). Note what learning and development you wish to undertake to support your personal development needs and your longer term development. SECTION B – THE NEXT 12 MONTHS This section should be completed by the reviewer following the review meeting and returned within in two weeks to the reviewee. PART 1 – SUMMARY OF PROGRESS Outline a brief summary of main activities, achievements and progress during the last 12 months and note any factors that have affected the achievement of previous objectives. PART 2 – AGREED OBJECTIVES AND CONTRIBUTION FOR THE NEXT PERIOD Outline the key objectives for the next review period and any actions that the reviewee and reviewer need to undertake to achieve those objectives. SECTION C – ROLE RESPONSIBILITIES AND ESSENTIAL TRAINING In this section the following topics will be discussed: Essential training i.e. any contractual, mandatory or compliance training that you have completed or must be completed. Your Job description. Please see the Reviewee Guidance for more information regarding the range of essential training that staff must undertake. If any essential training requirements are identified these must be noted in the Personal Development Plan (Section D) as part of the overall personal development plan. Note in this section any essential training that you have to complete. Please note if you wish to discuss your job description during the Review. Yes No SECTION D – PERSONAL DEVELOPMENT PLAN Development activities in this section can include; on the job training, one to one coaching, mentoring, personal skills and leadership and management development, improving IT skills, Welsh language courses, further qualifications, updating knowledge about the University, reading, project work, secondments and conferences etc. This section will also include actions to undertake relevant contractual, mandatory and compliance training as noted in Section C.
