Foundation Phase Profile Baseline (on entry) assessment Entry to Reception INSTRUCTIONS FOR TEACHERS SIMS Assessment Manager via Vale Learning portal 2015/2016 Introduction These instructions are to guide you through the process of entering data into SIMS Assessment Manager to record outcomes for the Foundation Phase Baseline assessment. A child must be assessed in line with the Foundation Phase Profile Baseline Assessment within 6 weeks of starting Reception (or Year 1 if they do not attend school in Reception). An outcome must then be recorded for 4 Areas of Learning (AOL), and for each of the strands within these AOL. For example within Personal and Social Development, Wellbeing and Cultural Diversity (PSD) an outcome must be recorded for Social interaction, Behavioural regulation, Response to others and Independence in personal care. The scores from each of these are then added up by the system and an overall level for the AOL should be recorded. The valid outcomes for the assessment are: 1 N (Not awarded an outcome) D (Disapplied) W (Working towards lowest outcome) Z (Bronze) S (Silver) G (Gold) 1 (Outcome 1) 2 (Outcome 2) 3 (Outcome 3) 4 (Outcome 4) 5 (Outcome 5) 6 (Outcome 6) A (Above Outcome 6) 1.
