details in your profile and all details are correct as per your SYID Details.
a.) Latest Passport size color Photograph and
b.) scan copy of Address Proof *
c.) scan copy of Photo ID Proof *
d.) Address
e.) Date of Birth
f.) Valid Mobile Number
g.) Valid Email ID
All details in Basic Info,Contact Info and Advance Profile tab should be uploaded and
complete.
Failing compliance with above Mandatory pre-requisites, your Shivir Registration will be
cancelled without any notification.

1. Enter your SYID
2. Select any of the three options to search by
3. First Name, Mobile Number or Date of Birth
4.
Select any one of the Verification method to reset password.
i.e.
a.) Email
b.) Mobile.
If mobile is selected and submitted then it will send verification code on your Mobile .It will
also show first 4 digits of your Mobile Number used at the time of SYID creation.

Steps to Upload Photo ID Details are as follows.:
1. Login with your SYID and Password on www.shivyogportal.com
2. To Upload scan copy of Photograph and Govt. approved ID card select advance profile tab
and upload details.
Address Proof : Ration Card, telephone bill,Credit Card bill,Passport ,Electricity Bill,Gas
Connection Bill, Aadhar Card,Bank Passbook,Driving License,Voter ID.
Photo ID Proof : Pan Card,Passport,Voter ID,Aadhar Card,Driving License,health
Card,Govt Issued ID Card
