Anna is very Young but will communicate her feelings using MAKATON and nonverbal communication. In future consideration needs to be given in advance of how

What are the key strengths?
Anna is a sociable, smiley, bright child who loves to swim and go out on her bike.
Anna also likes musical books and toys. Anna enjoys interacting with her peers.
She attends nursery five mornings each week which gives her the opportunity to
develop her communication and social skills. Anna is learning to use MAKATON to
communicate. Anna’s health has improved over the last two years and this has
assisted her developmental progress. Anna has a strong and positive attachment to
her foster carers
In which areas does the child need additional support?
Anna needs daily assistance with all aspects of her care due to the nature of her
disability. Anna also requires to attend ongoing medical appointments to monitor her
health and may require further surgical intervention in the future
Anna requires assistance in nursery to ensure she is included and to promote her
learning
Are there any risks / concerns for the child?
Anna was born at 32 weeks gestation with complex medical needs. Anna spent
much of the first year of her life in hospital and had subsequent periods of being
admitted to hospital. She has Myotonic Dystrophy which impacts on her muscle
functioning and also on Anna’s overall development.
What are the key outcomes sought for the child in this area?
Outcome 1

how these needs will be met

2. What I need from people who look after me
Key information:
Following Anna’s discharge form hospital she was in two different fosterplacements
prior to moving to live with Mr & Mrs Rennie
Evaluation:
What are the key strengths?
They provide a high level of care, emotional warmth and provided a consistency in
Anna’s life. Mr and Mrs Rennie provide a range of play opportunities for Anna e.g.
going out with Anna on her bike and spending time interacting with her reading.
Mr & Mrs Rennie attend all relevant appointments with agencies in relation to Anna
and will seek guidance and support as required. Mr & Mrs Rennie keep information
about Anna’s progress, photographs, and have historical information regarding her
birth family for the purpose of her Life story.
In which areas does the child need additional support?
Anna requires a very high level of care and consistency due to her disability. Mr &
Mrs Rennie would like ongoing guidance and support in helping them to look after
Anna and make decision regarding her future
Are there any significant risks for the child?
The long term outcomes for ANNa being a looked After child
Since Anna moved to live with Mr & Mrs Rennie there has been no contact with her
birth father (Liam Brady, whereabouts unknown) At present Anna has no awareness
of her circumstances, Mr & Mrs Rennie are considering the possibility of adopting
Anna in the future
Argyll and Bute Council currently have Parental Rights and Responsibilities in
respect to safeguard and promote Anna’s long term welfare

Key information:
Anna has lived in her current placement for over two years. During this time Anna
has attend playgroup prior to attending Argyll Angels nursery.
There is already a significant formal support network in place for Anna

What are the key strengths?
The family home is a secure environment and has space for Anna to move around.
Mr & Mrs Rennie have their own business which they operate form the family home
which enables them to work flexibly around Anna’s needs
In which areas does the child need additional support?
Anna’s paternal aunt, Katy Brady indicated she was willing to be in contact through
correspondence but her current address is unknown
Are there any significant risks for the child?
There requires to be a high level of communication to meet Anna’s future needs and
consideration also needs to be given to other formal sources of support as Anna gets
older and starts Primary school

Additional Information
Mr & Mrs Rennie are considering adopting Anna. Argyll and Bute Councilcurrently
have parental rights and responsibilities in respect of Anna. If Mr & Mrs Rennie wish
to pursue this, Social Work will complete the relevant paperwork and present the
case to the Permanency Panel within 3 months. It is also recommended Mr & Mrs
Rennie seek legal advice.
