Academic Eligibility 1. Pass 20 units – (4 classes) – Only 1 class can be in PE / Athletics Must be enrolled in 20 units 2. Overall Grade point average – 2.0 3. One probationary period allowed during the athlete’s 4 year career 4. Eligibility determined from the previous 9 week grading period Residence Eligibility Living within the boundaries of the school of attendance area Verification of residence may be necessary for eligibility Scholastically Eligable Athletic Clearance Current Physical Screening – good for 1 calendar year (must be completed and signed by Doctor). Insurance certification Completed emergency card & consent for emergency treatment Athletic Code of Conduct CIF Head’s Up Concussion Form Locker Room Policy BOYS AND GIRLS LOCKERS ARE AVAILABLE BUT STUDENTS MUST BE RESPONSIBLE FOR ENSURING THAT LOCKERS ARE LOCKED AT ALL TIMES. NEVER LEAVE YOUR BELONGINGS UNATTENDED OR LOCKERS UNLOCKED Complaint Procedure Personnel or program complaints 1. Initial complaint directed to the coach 2.
