Walk Across Tennessee 2017
A team of 5 people for 5 weeks!
Teams are not required to walk together.
The team captain does report team miles.
Here are the “steps” to Walk Across
Tennessee and important dates!
Step 1. Form a Team of 5: 1 captain & 4 members. All five walk or use conversion chart.
Members turn in your individual registration form to your team captain by April 5!
Step 2. Captain will distribute materials to team members.
Step 3. Captain will complete a Team Registration Form. Captains turn in all registration forms (including
team and individual registration forms) by April 7 to the Obion County Health Department!
Step 4. Each team member will keep an Individual Log Form. The captain and team members will record
their mileage on Friday’s each week. Captains will turn in team mileage Fridays. Start walking April 24.
New this Year!! Earn your team up to 50 Bonus Miles. Each team will receive 5 bonus miles for every
individual pre-survey turned in on April 7 with registration packet to the Obion County Health Department. The
same goes for post-surveys that are due on May 26. Your team can get 25 free miles by turning in the pre-survey on
time and 25 free miles by all your members turning in the post survey on time.
Step 5.
Step 6. Members will notify their captain each week with mileage total.
Step 7. Captains will record their own and their member’s mileage on the Team Captain’s Log, determine the
total number of miles walked by the team that week. Captains report your miles on Fridays to the Obion
County Health Department by calling 885-8722 Regina Reid or email her at [email protected]
Important Dates and Deadlines!
April 5 – Members turn in your individual registration form to your team captain by April 5!
April 7 – Captains turn in all registration forms (including team and individual registration, t-shirt forms and
t-shirt payment and pre-surveys) by April 7 to the Obion County Health Department!
Start Walking Monday, April 24.
Captains report miles on Fridays, April 28, May 5, May 12, May 19 and May 26 to the Obion County
Health Department.
Stop Walking May 26 (end of the day). Captains turn in final team mileage and turn in post surveys!
June 1 Attend the Walk Across TN Awards Reception on Thursday, June 1 from 4:00-6:00 pm at
the Obion County Public Library in Union City!
