Lake County Community Health Improvement Partnership (CHIP) 700 South J Street Lakeview, OR 97630 AGENDA CAC/CHIP Physical Activity Workgroup Meeting Minutes Date and Time: Place: 2/5/2014; 4:00pm – 5:30pm Lake District Hospital, South conference room Partnership members, staff, and guests in attendance: Mary Wilkie, Tallulah Chiono, David Kerr (by phone), Jamie Davis, Emma Cornell (by phone), Deb Diment (by phone), LuAnn Anderson, and John Adams. I Physical Activity Workgroup 4:00pm II Business 4:15pm John Adams provided a review of the (informal) agenda and the last meeting's minutes. John provided updates from the RCAC meeting in Boardman, including the Communtiy Health Improvement Plan deadline extended to April 18th and the funding process beginning for proposals, including LOIs due March 7th. III Brainstorm Strategies 4:25pm The group worked to finalize strategies related to the problem statements: 1. Lake County underutilizes existing facilities for affordable, organized physical activities. This is a long-term project to establish an activities/recreation nonprofit or parks and recreation district with facilities and programming Next step is to form a committee to establish an activities/recreation nonprofit or district (inviting Ann Crumrine and Amy Hussey) Pursue a technical assistant grant for a master plan with The Ford Family Foundation North Lake needs to create a committee and identify/develop their strategy 2. Lake County has a lack of year-round low cost physical activity opportunities, which helps to contribute to high risks of heart disease. Lake County CHIP will be the lead organization to provide health education, promote physical activity and increase participation in existing physical activity programs. The effort will include: Develop an inventory and assessment of existing and available year-round physical activity programs and services within the county, including participation rates - Lake Lake County CHIP Coordinator: John V. (541) 947-2114 Ext 375 | Cell (541) 219-0907 | [email protected] Lake County Community Health Improvement Partnership (CHIP) 700 South J Street Lakeview, OR 97630 IV County Public Health will conduct the survey Develop a website that exists as a centralized resource for the community about available physical activity opportunities within the county at any given time Enhance the existing community walking routes within the county by gaining County and Town recognition of formal community walking routes and establishing at least one (and possibly multiple) kiosks on the walking routes. Additionally, maps of the walking routes will be created and disseminated throughout the county and posted on the community activity website. Physical activity health education and outreach will be conducted throughout the program year The goals are to educate the community about the benefits of physical activity and the consequences of inactivity, increase awareness about available and existing physical activity opportunities, to increase the overall participation in existing programs and services, and to lower the rates of self reported risk factors for heart disease Another strategy includes proving more access to physical activity opportunities to lowincome individuals with risk factors for heart disease by offering scholarships to interested and eligible participants Providing 50 scholarships, partnering with Lake County Medical Clinic, Warner Mountain Medical Clinic, and/or North Lake Clinic and willing physical activity programs and services The goals of the scholarship program are to provide more access to physical activity programs and services to low-income and individuals with risk factors for heart disease. Another goal will be to lower the rates of self reported risk factors for heart disease.
