This is a take-home exam distributed during orientation. Audiology Writing Assessment. This is a take-home exam distributed during orientation. Formative Assessment of Clinical Skills. Students will receive a copy of this exam during orientation and will have opportunities to practice all skills prior to each exam. Audiology Formative Skills Assessment. This is completed by clinical preceptors at the end of each semester. Students will be able to see their clinical strengths and weaknesses and to address these areas in subsequent clinical placements. Institutional Review Board for the Protection of Human Subjects. The thesis advisor will explain to the student how to complete this form prior to data collection for the thesis project.
