Parental consent form for the use of photographs, video and web
The Black Country School Games School games organising committee recognise the need to
ensure the welfare and safety of children and young people. In order to comply with the Data
Protection Act 1998 photographs, videos and filming for use over the internet of children and
young people will not be taken without the consent of the parents or carer.
