ADVANCED SPIRITUAL CARE
This interactive presentation focuses on advanced spiritual care techniques for faith community
nursing practice, but is applicable to any nursing practice setting. While the tools and techniques
discussed are not unique to nursing spiritual care, they have been customized through the
application of a nursing process framework. Participants will receive a copy of the Power Point
and teaching outline for use in their practice.
Objectives:
1. Distinguish between religion and spirituality.
2. Describe the link between spirituality, health, and spiritual care.
3. Evaluate the need for caregivers to practice personal spiritual care.
4. Identify spiritual care practice options including assessment tools for individuals and groups.
Prayer: Where Two or Three are Gathered
This interactive presentation explores the concept and meaning of prayer from both personal and
professional perspectives. Research about prayer, benefits, and using prayer as an effective
nursing intervention are included. Participants will receive a copy of the Power Point and
Prayers for the Soul: Comfort for Parish Nurses & the People They Serve.
Objectives:
1. Explore the concept of prayer.
2. Discuss current literature and research involving the benefits of prayer.
3. Examine various types of prayer including potential barriers.
4. Investigate how to use prayer as an effective nursing intervention.
Closing the Gap: The Faith Community Nurse Role in Transitional Care
Evidence continues to demonstrate that when patients transfer from hospital to home, a critical
gap in the continuity of care often exists. The specialty practice of faith community nursing
(FCN) bridges and closes this gap. This interactive presentation will guide FCNs to apply skill
sets that are both proven transitional care strategies and intrinsic components of practice. By
heightening wholistic healthcare methods already utilized by practicing FCNs and customizing
assessment skills for risk factors, interventions and disease management focused on transitional
care, participants will apply best-practice standards to promote the integration of FCN practice
into transitional care as a bridge between hospitals and communities of faith.
Objectives:
1. Identify factors that place patients at risk for readmission
2. Apply transitional care research findings to current FCN practice
3. Demonstrate an understanding of transitional care factors in the nursing process
4. Discuss the application of transitional care strategies in personal practice
