Closing the Gap: The Faith Community Nurse Role in Transitional Care Evidence continues to demonstrate that when patients transfer from hospital to home, a critical gap in the continuity of care often exists. The specialty practice of faith community nursing (FCN) bridges and closes this gap. This interactive presentation will guide FCNs to apply skill sets that are both proven transitional care strategies and intrinsic components of practice. By heightening wholistic healthcare methods already utilized by practicing FCNs and customizing assessment skills for risk factors, interventions and disease management focused on transitional care, participants will apply best-practice standards to promote the integration of FCN practice into transitional care as a bridge between hospitals and communities of faith. Objectives: 1. Identify factors that place patients at risk for readmission 2. Apply transitional care research findings to current FCN practice 3. Demonstrate an understanding of transitional care factors in the nursing process 4. Discuss the application of transitional care strategies in personal practice
