CDS M.S. TENNIS TEAM

Dear Parents,
Your child is interested in participating on the CDS Tennis Team. Your child MUST have the basic
skills of serving, forehand and backhand and be at an advanced beginner level or higher in
order to come out for the team. Please note: we play 5 singles and 2 doubles matches; players
will be seeded on a ladder for ranking 1-9. Depending on the size of our team there may be a
chance that a player does not play every match. Players may participate in both Track & Field and
Tennis.
There is a $225.00 athletic fee to participate in this sport. CDS will bill you for the fee through FACTS.
CDS will provide a team jersey to be worn at matches and returned at the end of the season. Athletes
will need to provide black shorts, skorts or skirts for matches. The bus will drop off the team at
matches and practices & parents MUST pick up the players at the site.
All participants must have the following forms on file:
• FHSAA Preparticipation Physical Examination [Grades 6 - 12: due annually before practice
starts or expiration of previous form]
• FHSAA EL3 Consent and Release Form (four pages) [Grades 6 - 12: due annually at the start
of every school year, before student's first day of practice]
• CDS Athletic Authorization/Consent for Disclosure of Protected Health Information [Grades 6 12: due annually at the start of every school year, before student's first day of practice]
• CDS Sports Conduct Form [Grades 4 - 12: due annually at the start of every school year,
before student's first day of practice - students must sign also]

Our tennis scores are based on 5 singles matches and 2 doubles. Before each match we will try to
find out how many players the opposing team will have and then travel with as many so that we can
play as many tennis matches as possible. We will try to have a “list” of traveling players for each
match when our season begins. We usually start with singles first then as one court empties the next
match goes on (we should have at least 6 courts available for each match). Match time varies but an
average time is about 45 minutes/match. We do play an 8 game pro set; no add scoring (first to 4
points wins the game), and a 7-point tie-break (win by 2) if a match is tied at 7-7.
In order to participate with the tennis team, students and parents should understand eligibility
requirements and information.
• Middle school students must have a 2.0 GPA, or the equivalent of a GPA based on a 4.0 scale, at
the conclusion of each semester. Ineligible athletes will not be able to practice or play with the
team.
• Athletes must demonstrate a cooperative, motivated, well-behaved manner at all times (i.e., at
school, during practices, and traveling with the team and on the playing field).
• Team members will show respect for, be considerate of, and display a positive attitude towards
teammates, coaches, referees, opposing players, and parents at all times.

promotes self-discipline, group cooperation, and good sportsmanship.

there will be NO transportation back to school, you must pick up at the park). Pick-up time is
