In volunteering to participate in this basketball game, I agree that, to my knowledge, I have no limiting
physical conditions or disability that would hinder my full and safe participation in this event.
I knowingly understand that my participation in the aforementioned activities may expose me to certain
foreseeable and unforeseeable risks of damage and/or bodily injury.
I knowingly, freely and voluntarily assume all risks and engage myself in the participation of the
basketball alumni game at Sherman Indian High School.
I hereby release Sherman Indian High School from any and all liability arising out my participation of the
above mentioned activities and hereby waive my rights herein to assert any claim(s) for damages, bodily
injury or serious bodily injury to the fullest extent allowed by law.
