PQAP Reaccreditation for IIS Professional Members Guidelines and Reaccreditation Application Form This documentation outlines the process for reaccreditation for current IIS Professional members. Background Sport Ireland Institutes Professional Quality Assurance Programme (PQAP) was established in 2007 to assist the Institute in ensuring that all Institute athletes are supported by service providers of the appropriate quality, i.e., the best practitioners work with the best athletes. IIS professional membership is valid for a period of 5 years, so we are now asking all registrants to complete the reaccreditation process if they wish to continue as a full member of PQAP. Re-accreditation Service providers applying for reaccreditation will be required to show that they have: 1. 2. Continued to work as a service provider in the field of sports performance, throughout the last 5 years. Maintained and advanced their level of knowledge in all appropriate areas, through CPD and a. kept an up to date and accurate record of all relevant CPD b. demonstrated that CPD activities are a mixture of learning activities that are relevant to current or future practice (see attached examples); c. ensured that the CPD has contributed to the quality of their practice and service delivery. *As of October 2014, all Strength and Conditioning reaccreditation applicants will be required to have, and show proof of, UKSCA accredited membership also. To demonstrate this, we will be asking service providers to submit the following, along with the reaccreditation application form: 1. Curriculum Vitae (CV) The CV template is named as Appendix 1. Please complete all relevant sections. Ideally CVs should be kept to 1 page. 2. Statement The statement template named as Appendix 2. The statement is a maximum of 500 words, describing your practice history and how you have developed as a sports science professional over the last 5 years. The statement should help to show the CPD assessors how your CPD activities are linked to your work. It should also help to show how your activities are relevant to your current or future work. 3. CPD Profile The profile template is named as Appendix 3. The profile is a list of your CPD activities from the last 5 years that is relevant to your professional practice and a summary of learning from each activity. 1 Your CPD profile must demonstrate a mixture of learning activities that are relevant to your current or future practice. Your CPD profile should demonstrate CPD activities during at least 3 of the previous 5 years (i.e., your examples should be spread out over the 5 years and not all concentrated in one year). 4. Professional Declaration On section 5 of the application form we will be asking you to sign a professional declaration. You will be signing to state that you have: continued to practice in my stated discipline since my last registration; continued to meet the competencies for the safe and effective practice of my profession, including following the IIS Professional Membership Code of Conduct; If you cannot sign any part of the declaration you should contact the Institute in writing, explaining your circumstances. When completing your application for re-accreditation, you will need to refer to: Professional Member Code of Conduct Process and Timelines Each reaccreditation application is assessed by an assessment panel (a sub-group of the PQAP Committee). Applicants will be informed by e-mail of the outcome of reaccreditation normally within 8 weeks of the deadline for receipt of applications. We would be grateful if you could confirm, via e-mail, if you are planning not to apply for reaccreditation. Within 5 working days of receipt of each application, the applicant will receive an acknowledgement by email of receipt of the application. Each re-registration application is assessed by two assessors. Applicants who have met the reaccreditation requirements will be notified that they can continue to remain on the PQAP register. Applicants who fail to demonstrate that they have met the reaccreditation requirements will be asked to submit further evidence and the application will be re-assessed. Applicants should be aware that they can be asked to verify all submitted examples of practice or CPD should the assessors request it. The fee for re-registering is €100. This fee covers the assessment of your application. This fee is nonrefundable. Payment can be made by bank transfer only to Sport Ireland. (Please ensure to include your name on the bank transfer for reference purposes). Key Dates The yearly deadlines for all applications are March 01st and September 01st. 2 If the applicant misses either of these dates they are still permitted to apply for reaccreditation up until the expiration of their application. Their accreditation will be temporarily extended pending the outcome of the review process carried out at the next available round of applications. Should an accredited service provider fail to apply for reaccreditation by the expiration date of their accreditation they are permitted a period of 1 year from the expiration deadline to apply through the reaccreditation process. The service provider’s name/contact details will be removed from Sport Ireland Institutes website as an accredited service provider until they apply through the reaccreditation process. Once the accreditation is more than 1 year expired the individual will be required to reapply though the initial accreditation application process. PQAP accreditation is valid for a period of five years. As such the process of re-accreditation is for use by applicants who are already PQAP members and is required every five years.
