GENERAL/LOBBY AREA
Are the required licenses currently displayed in a visible area
Are Exit signs lit, visible and free from any obstruction
Is the floor free form any loose and/or dangerous materials
Is the Defibrillator visible, easily accessible, and free from any obstruction
Is there adequate amount of lighting; fixtures in good working order
Are all electrical cables hidden and/or placed where they are not a danger
Are the computers/phones in good working order
Is the first aid easily accessible and fully stocked
are the fire extinguishers in place and in good working order (not expired)
Is there a list of emergency contacts posted and current (preferably by the phone)
Is the room clean, in safe and orderly condition (nothing obstructing exit)
Are the smoke alarms/heat detectors in place unobstructed/ in good working order
Is the emergency/fire escape routes clear and unobstructed
Is the maintenance closet securely locked and doors in good working order
Does everything seem to be in good working order (if not please specify in notes)
GYM
Are all gym machines/equipment in good working order (if no please specify in notes)
Are the exit doors free from any obstructions
Are the TV’s, lights and Fan’s in good working order (if not please specify in notes)
