This induction pack is to help you as a learner to familiarise yourself with the team. It provides you with the basics about the team and should
help you to plan to meet your learning needs.
We recommend that all learners contact the team in good time prior to placement to identify who will be supervising your learning and confirm
start times/hours.
During your placement you should have a minimum of a preliminary, midpoint and final meeting with whoever is supervising your learning. You
should also undertake some form of leaning analysis then produce a learning contract and or action plan that provide structure and direction to
the time you are with us. Your supervisor will be able to help draw this together but you must take a lead in negotiating your plan. Some
examples of these tools are provided but you may wish to use those produced by your education provider.
Please ensure that you present any documents regarding your placement with our staff on the first day of your placement. This should include
any assessment document, learning outcomes you hope to achieve and any type of ongoing record of achievement.
On completion of your placement you will be asked to provide an evaluation. Please take time to complete this openly and honestly as it is only
with your feedback can we improve the quality of these valuable learning experiences.

Stafford Community Mental Health Team (CMHT) is a Multi-Disciplinary team which provides secondary mental health services
to the residents of Stafford and the immediate surrounding areas.
The team consists of practitioners and clinicians with a range of professional backgrounds from both Health and Social Care.
Staff are managed by an Operational Team Leader. Professional lines of accountability and support exist within all staff groups .
The service is for adults from the age of 16 who experience a range of functional mental health conditions and associated social
problems. Services are available for individuals (including their Carers) who require mental health assessment and care form
specialist mental health practitioners. (Excluding dementia).
Services are accessed through the Team Single Point of Access and a local Duty System. Operating hours are Monday to
Thursday 8.30 am to 5.00 pm and 8.30 am to 4.30 pm on Fridays. Bank Holiday excluded.
Referrals can be made by post, fax, telephone or email.
All referrals received by the team are triaged by the duty worker to ensure the most appropriate response to the referral.
Interventions and treatments for some service users may require shorter periods of care from the team. Where this is indicated
and when the agreed care plan/pathway and recovery goals have been reviewed/reached, at discharge care will be returned to
GP care. When agreed as appropriate with the service user referral on to other partners including third sector agencies will be
made.
Service users with complex needs may remain involved with the team for on-going treatment, care and monitoring for several
years. Care will be regularly reviewed with service users and the multidisciplinary team, through the Care Co-ordination
process. The team will work collaboratively with service user, their carer and other partners to support and outline a clear care
plan/pathway, to promote and enable improvement in the service user’s condition, quality of life and their identified recovery
goals.

Including individuals experiencing a dual diagnosis of either learning disability or substance misuse.
The service embraces working collaboratively with service users and their carer’s, agreeing recovery focussed care and care
planning. The team are committed to Social Inclusion and meaningful activity as an integral part of recovery.
The staff seek to ensure that all those involved in providing the service shall acknowledge and respect service user and carer's
gender, sexual orientation, age, race, religion. Culture, lifestyle and values. The service will be non-discriminatory and antioppressive by way of service delivery
The service operates within a quality framework upholding the Trust’s “Living our Values Framework”
Systems are in place to ensure that quality standards are monitored and non compliance is identified early and addressed.

appropriate to your level of training. Your supernumerary status is assured, and the team recognise your learning needs, and
will help you to identify learning opportunities to meet the objectives you identify for your placement.
You are expected to participate in all aspects of clinical care during your time with us, developing your clinical skills, whilst
maintaining and enhancing the high standards of care we strive to deliver.
During and at the end of your placement; we would welcome your feedback, views and any suggestions to enhance the training
environment/opportunities for future students on placement with the team.
If at any point during you placement you are unclear of what is expected of you or you identify or have a difficulty/problem that is
impacting on your training or placement we would welcome and encourage a discussion and feedback with your mentor and if

Student Nurses will have an opportunity to spend time with all the different disciplines / staff groups within our team but
will spend the majority of time with their Nurse Mentor.
Students will be able to follow the whole patient journey, from receiving referral, assessments, interventions by our team
and other teams or services as required and through to discharge.
Delivery of nursing assessment, care, treatments, Pathway through services, review and discharge within a community
setting.
Additional opportunities may include:
delivery of treatments by Nurse Therapist including EMDR, EFT, CBT, CAT skills, Nurse Prescriber – management and
prescribing of medication. Administration of psychotropic medication via an I.M Depot injection. Service User group
based work - Bi-Polar Education and Keeping Well Programmes.

The frameworks and
models of care used
The Stafford Mental Health Team offer the following services:
Screening of new referrals, prioritising those needing crisis, urgent or routine assessment based on risk and need.
Assessment, risk assessment, diagnosis, Formulation and care planning, prescribing of medication and its monitoring.
A choice of evidence based interventions including Cognitive Behaviour Therapy, Personal Centred Counselling,
Motivational Therapy, EMDR, EFT, Nurse Prescribing Service, Occupational Therapy,
Social Care Assessments. Social care interventions including care packages which meet support people mental health
needs allowing them to continue to live in their own homes within the local community. Statutory Mental Health
Functions.
Care programme approach (CPA) underpins the care delivered by the team within a clinical governance framework.
CPA provides for a systematic assessment, needs led and comprehensive approach to care planning and risk
management. Clinical Governance is at the heart of effective services - everyone working in the Trust has a
responsibility to help develop and deliver quality services for patients and carers.
The Stafford CMHT operates within a recovery focused framework. The service is based upon a Bio-Psycho-Social
Model and recognises the importance of linking physical and mental health needs, working with statutory, non-statutory
and third sector services to ensure a ‘whole health’ approach.

& Security
As with all policies, the Lone working Policy can be found on the Trust intranet.
Local lone working guidelines will be discussed as part of any induction process.
Some of the local guidelines include:
On entering Foundation House, reception staff will greet visitors; the door into the main building is restricted by a key
code lock. Do not allow any unidentified persons through this door without clarification of who they are (ID badge if
appropriate) and their business/reason for entering the premises.
Staff complete a weekly diary sheet, which outlines their non clinical meetings and clinical contacts – this is updated
should there be changes.
Staff use the team in/out board which is located in the team duty room, when not returning to the office they make
arrangement to advise of their safety at the end of their working day.
Staffs have mobile phones and are contactable when out of the office and likewise they can contact the team base.
Where risks are identified Staff will consider the venue where clients are seen and be accompanied by colleagues as
appropriate.
Expectations of students
Students are expected to be punctual, and advise staff should there be a problem with arriving on time.
Students will inform their mentor and/or the team and university if they are unable to attend through sickness or any
other reason, this will completed as soon as possible on the day and subsequent contact as agreed to update if nonattendance on placement (regardless of reason) is indicated.
Students will keep a weekly diary sheet, identifying where they are and who they are with if not working with their
mentor, this will be updated should there be changes.
Students will use the team in/out board which is located in the team duty room, to identify where they are and who they
are with, when away from the team base.
Students will always conduct themselves in a professional respectful manner.
Students will uphold the rights of the service users they meet and have a clear understanding and demonstrate respect
for dignity and equality.

Students are advised that this a non uniform placement and respectfully requested to wear appropriate clothing and
footwear, and to clarify with their mentor if uncertain. Please see the University and Trust Policies.

Housekeeping
The CMHT is located on the first floor of Foundation House.
Students will be familiarised and orientated to the building on arrival and during the first few days of their placement.
Fire assembly point is in the car park at the front entrance to Foundation House. There are a number of fire
extinguishers and exits throughout the building; students will be made aware of these during their induction and
orientation to the building.
There is a lift from the ground to first floor available should this be required.
There is a disabled toilet on the ground floor.
There is a small kitchen with kettle, fridge and microwave oven for staff and student use.
There are various food outlets in the town centre a short walk away.
