By entering this tournament, we agree to follow all rules and hereby waive and release all contestants,
sponsors, and all tournament officials and organizers from all claims of injury and/or damage incurred in
connection with this tournament. I understand the entry fee will not be returned after the deadline date.

Park fees apply. Ensure you have a parks pass or cash on hand to purchase a one-day pass.
All GFP Rules and regulations apply to include Orman Damn fishing slot.

tournament. Your confirmation of team entry and team number will be sent to the email address provided on the application.
If there is a problem with your entry form we will contact you. Please make sure to have a paper copy or easily accessible
electronic SD Fishing License on hand during the tournament as SD GFP Game Warden(s) will be on site.
