Our Commitment
The Dr Alistair Partnership is committed to
providing an excellent standard of service
and healthcare. We also undertake, as a
teaching practice, to pass on our expertise by
the training and development of fellow health
care professionals.
We accept patients from Bryn, Ashton,
Golborne, Lowton East and Lowton West.
Please be aware that we are a teaching
Practice and at times have medical students,
trainee GPs and student nurses. You will be
advised prior to consultation if they will be
present. If you do not wish them to be present
during your consultation please inform
reception.
As a patient, you have a right to express your
preference of practitioner when booking an
appointment. The Administration and
Reception Team will offer every assistance to
identify your best source of help
and course of action.

Registration
To register as a patient with us, please contact the
Partnership on one of the numbers listed on the
cover. If you wish to register with us you will be
asked to complete a registration form and health
questionnaire. You will be invited to attend a new
patient health check with a friendly Health Care
Assistant.
As a patient, you can expect to…
> be treated with courtesy and with respect for your
privacy and dignity
> receive treatment on the basis of your clinical
need
> be able to choose to have a relative or friend with
you during consultations and examinations
> be informed on arrival at an appointment if delays
are occurring
> take part in all decisions about your treatment
and to have the pros and cons of treatment,
including any risks, side effects and alternative
methods of treatment, fully explained
> have details about you, including your medical
records and anything you say, treated in
confidence and, except where required by law, to
be given an opportunity to decide whether your
details can be passed to anyone else
> be given access to information kept about you
> know the names and professional status of all the
staff involved in your care
As a patient, you have a responsibility to…
> treat the staff who care for you with courtesy and
respect at all times
> inform the Practice of any change in contact
address or telephone number
> attend appointments on time, or give reasonable
notice of inability to attend
> consider the consequences of refusing treatment
or not following medical advice and accept
responsibility for your own actions
> let the staff know if you have any allergies or
sensitivities to medications
> take any medicines as instructed and seek
medical advice before stopping or changing
treatment
> treat other patients with courtesy and respect
Repeat Prescriptions
Repeat prescriptions can be ordered by contacting
the surgery after 10:30am, or dropping off a request
in the prescription box at the surgery. Please allow
48 hours for prescriptions.
The Practice sometimes uses personal information
to assist in teaching medical students
We will not disclose personal information in any
other situation without your explicit permission,
unless there are exceptional circumstances, such
as when the health and safety of others is at risk or
if the law requires us to pass on information
Other Primary Care Services
Complaints Procedure
Complaints may be lodged verbally, or in writing, to
the Practice Manager. If the Practice Manager is
unavailable at that time, please leave your contact
details with a member of the reception team and
the Practice Manager will contact you as soon as
possible.

Open from 7.00am in the morning to 9.00pm at
night, seven days a week.
Abuse
Throughout the Partnership we operate a policy of
Zero Tolerance, where abuse of any kind towards
members of staff is unacceptable. Any person who
uses threatening behaviour, physical or verbal,
towards staff or members of the public in the
surgery will be asked to leave and the police will be
contacted.

Open Monday to Saturday 7.00am to 10.00pm,
and Sundays 9.00am to 10.00pm.
Confidentiality and your Health Records
We make every effort to keep patient information
confidential and secure. When you register with us,
we create a new health record for you. We also
obtain your past health records from your previous
GP Practice.
The Primary Health Care Team (doctors, nurses,
health visitors, district nurses, midwives, managers
and support staff) use your information to provide
effective healthcare. The Practice may also share
personal information with other people involved in
your care, such as hospital doctors.
