About Easter Seals Blake Foundation
Intake & Coordination of Care Services
Intake and Coordination of Care Services provides children, teens and adults with customized eligibility, enrollment
and referral services for covered behavioral health needs including therapy, family support and in-home intervention.
Comprehensive assessments, coordination of treatment plans, as well as, psychiatric and medication services are also
available.
Children & Family Services
Children & Family Services programs provide support to children and families in the areas of health, child abuse and
neglect, parenting, child development and early education, behavioral health and developmental disabilities.
Community Living Services
Community Living Services provides residential and rehabilitative care for adults with cognitive & physical
disabilities. Some of the residents are dually-diagnosed with traumatic brain and spinal cord injuries. Their
disabilities have led to self-care deficits that require them to need full-time habilitation care in order to perform daily
living activities.
SAGE Employment & Community Services
SAGE provides individualized residential supports and job development services to individuals with disabilities who
are in their late teens through adulthood and operates with a philosophy based on the belief that individuals with
disabilities must be full and active participants in school, work and community occupying socially-valued roles.
SAGE also offers Behavioral Health Services, including Equine Assisted Mental Health services to youth.
Supportive Living Services
Supportive Living Services provides therapeutic level II group homes for children aged 5-18 with behavioral and/or
mental health issues, as well as, level II group homes for adults 18 and older with serious mental illness, traumatic
brain injury, and/or substance abuse issues, plus outreach services to ensure medication compliance, and daily living
supports in community based settings.

We will provide opportunities, supports and services to children and adults with disabilities and their families,

based on their own choices.
FUNDRAISING EVENTS
Cooking With The Stars
October 27th, 2016 at Mr. An’s, Tucson, Arizona
$175.00 per person
The dinner is a special evening with 180+ guests, where local athletic and entertainment celebrities “host” the
event as personal chefs. The evening includes dinner, a silent and live auction and some entertainment for all
our guests to have fun.
Walk-Run-Roll
Saturday, May 6th, 2017 at Reid Park (22nd and Country Club)
The Walk-Run-Roll event is an opportunity for the community to come together to raise funds for Easter Seals
Blake Foundation. The walk will be a family friendly event that includes a 2.5K leisurely Walk and 5K Run,
followed by an expo, food and snacks and entertainment. We are expecting in excess of 400 walkers that day.
Spring Golf Classic
May 19th, 2017 at Sewailo Golf Course at Casino Del Sol
Individual golfer $150.00
Foursome $600.00
The Spring Golf Classic is an annual event to benefit Easter Seals Blake Foundation. The event includes the
golf tournament, followed by a buffet dinner with a raffle and prizes.

Dear ESBF Walk Participants,
Welcome to our Eighth Annual Walk Event! On behalf of Easter Seals Blake Foundation and the children
and families we serve, I would like to thank you for your support.
Our mission is to enable each individual served to discover and meet his or her maximum potential for
independent, productive living and developmental growth. Easter Seals Blake Foundation’s work in
the community has evolved since 1950 and today we offer a wide range of programs and services
throughout Southern Arizona through our Community Living Services, Children & Family Services,
Supportive Living Services, SAGE Employment and Community Services, and Intake &
Coordination of Care Services divisions.
Every dollar raised for the organization goes directly back into our local community, providing
programs and services to more than 22,000 children, adults and families each year. We strive to
supply the people we serve with the support, knowledge and tools they need to live healthy and
productive lives. Your generosity helps ensure the availability and success of our services.

BUILD YOUR TEAM!
Getting Started
 Set a goal. How many walkers do you want to recruit and what is your fundraising goal?
 Create your team web page at www.walk-run-roll.com.
 Invite friends, family and co-workers to be on your team. If you are coordinating a corporate
team, invite people located in different departments or work sites. They can also invite their
friends and families to join the team. ANYONE can be on your team!
 Put Walk-Run-Roll brochures/posters in centrally-located, well-traveled locations, and let coworkers know where they can get one. Be sure walkers contact you directly so you can keep track
of your team members. Keeping track of your team online is the best and easiest way, so
encourage online registration!
 Place a Walk-Run-Roll article in your organization’s newsletter. A sample article is included in
this kit for you to personalize. Check now to find out your newsletter’s deadline.
 Try to get on the agendas of all department meetings. If you would like a staff member from
Easter Seals to come speak to your co-workers, contact Carrie Durham at 520.327.1529 x124 or
[email protected]. This will help recruit your team!
 Find out if your organization provides matching funds for charitable donations. Look on your
organization’s website or contact your Human Resources manager and ask whether or not the
organization will match your donations.
 Use social networking sites such as Facebook and Twitter to announce that you are recruiting team
members.
Networking Works
 Determine the best communication options for your organization. Use email, memos, paycheck
stuffers, bulletin boards, intranet, social networking sites, blogs and word of mouth to let everyone
know about Walk-Run-Roll. Sample email messages are included in this kit.
 Think of other ways to get people involved. Involve management! Promote friendly competition
between departments or plan an informational meeting or booth in the lunch room to get the word
out to everyone. Set up a laptop and register walkers online at www.walk-run-roll.com.

Team Building and Fundraising Events
 Host team fundraisers such as 50/50 drawings, car washes and yard sales. Advertise these events
on your team web page!
 Have each team member send out personal emails with the link to donate online.
Keep Your Team Energized
 You need the full attention of your team. Continue regular contact via email to maintain team
energy until the event.
 Encourage team members to turn in donations. This will help validate all the hard work they’re
doing while creating a friendly competition amongst the team.
 Hold a special team meeting to get energy flowing and brainstorm ideas to get more donors.
Building Team Spirit
 Plan a meeting to coordinate details. Send around a memo about the Walk meeting.
 Give an extra copy of the Walk donation form to each team member. Encourage them to ask
people – family members, friends, co-workers and neighbors – to join them on
May 6th, 2017.
 Determine a good meeting spot for your team on the day of the Walk. Discuss car pooling and
maybe even a team lunch afterwards.
Rounding The Bend
 Check with key team members about meeting your goals. Plan one last organizational sweep.
Members should be encouraged to check online donations for accuracy in keeping track of the
team goal.
 Check that all team members have donation and matching gift forms, if applicable. Have extras
for latecomers.

Ideas for Fundraising Success
Personalize your fundraising request with information about yourself and your history with Easter Seals
Blake Foundation. Use the information provided at www.walk-run-roll.com to inform people how the
funds raised for Easter Seals will be used and why it’s important to you. Track donations online.
Be sure to send a thank you when you receive a contribution!
Important Notes:
There is no fee but there is a fundraising minimum of $5.00 for all participants who register in advance.
Walkers (ages 13+) who register the morning of the event (May 6th) will be asked for a minimum
contribution of $25.
What do I do with cash and checks from people sponsoring me in the Walk?
Donors should make checks payable to “ESBF”. Write your team name in the memo section on all
donor checks so you are properly credited, and bring these donations to the event itself. If you receive
donations after the event, you can mail these contributions to:

Couples who file jointly can reduce their state taxes by up to $1,000.00; individuals or head of

Each person who registers for ESBF Walk can create a personal web page for fundraising. Log in
at www.walk-run-roll.com and click on “Register Here” on the left to access fundraising tools
online.
Make Fundraising “Fun” With These Online Tools

Your Online Headquarters (HQ) has easy-to-use tools for you to import your email
address book and send email to friends and family. We even provide suggested text
and customized links to your personal Walk Web page! You can also customize your
personal Walk web page with text, a photo and a blog. We’ve made it easy. Check
it out!

Friends can make secure donations online while visiting your personal Walk web
page. Ask them to visit www.walk-run-roll.com and search for your name. They can
view your individual or team Web page and make a contribution toward your walk
efforts. Easter Seals Blake Foundation will acknowledge each online gift and send a
thank you email that can be used for tax purposes.
There are downloadable and printable documents in Adobe PDF format. You will need Adobe
