When to use this form
This form is to be completed by candidates who are applying for promotion to senior lecturer,
senior research fellow, associate professor, associate professor (research), professor and
professor (research).
How to complete this form
Candidates complete the form in accordance with the Academic Promotion for Candidates
Levels C-E Procedure.
Please refer to the Monash HR academic promotion website.
Candidates must:

not alter set margins, fonts or font size defaults;

not delete any irrelevant columns or tables unless instructed on the form; and

use A4 black and white pages (or colours compatible with copying).
How to Lodge Applications
Australia:
Lodge completed and signed application, along with attachments (specified in the procedure)
online by the closing date of the promotion round.
Malaysia:
Lodge completed and signed application with the following additional documents for Malaysian
based candidates (please refer to the procedure):
Application for Promotion Cover Page: Malaysia Levels B-D
Malaysia: Promotion Research Achievement Record Form
Lodging applications:
Levels C and D: email all required documents to the Malaysia promotion coordinator by
the round closing date.
Level E: Lodge completed and signed application, along with attachments (specified in the
procedure) online by the closing date of the promotion round.
