Uniform Ordering Instructions

1. To order your uniform, visit www.lookofassurance.com
Are You Eligible for a Uniform Credit?
If you are a full-time IU Health Team
Member making less than $33,146 per
year, you are eligible for a $60 uniform
credit. This will automatically be updated
to your account, and you will see the
credit when you log-in.
2. Use the following Log-In and Password to access the website:
• Log-In:
IS + 5-digit Employee ID# (i.e. IS12345)
• Password:
Your first initial capitalized + the first initial of your last name lower case +
Uniform + last 4 numbers of your social security # (i.e. JsUniform1234)
3. If you have a Uniform Credit Certificate to redeem, click on the “Redeem Uniform Credit” at the top
of the page and enter your Certificate Number. This will automatically put $60 in your account.
4. At the bottom of the home page, click on “Shop IU Health” or “Shop at Riley” to choose the proper
logo for your uniforms.
5. Click on the “Inpatient” or “Physician Offices and Out-patient Clinics” or click on the pull-down
menu on the left side of the page. This ensures you are ordering the correct uniform.

6. Click on your job title.
7. Choose your uniforms and add to cart.
8. You may select to pay for your order with a credit card or payroll deduct.
9. Your Order Acknowledgement will be emailed to the email address you provided in your Profile.
10. Your order will be shipped directly to your home. See your Order Acknowledgement for more
information on approximate delivery dates.
