The idea
HYHM is about opening up information access
so people can make an informed choice of
what self-management options exist ,and
where, and how to get engaged.
The idea, simply, is systematising access
based around web-based sources rather than
relying on ad-hoc networks
Who benefits and why
Self-Management Support fit
All of the Franklin population that needs access
to information about available health & social
care resources, including activities and support
groups/networks.
