By signing the above, I understand and agree to release ALL of
the sponsors, organizations, and facility, for any injury, harm,
loss, misadventure, or inconvenience to myself as a result of
taking part in Chruch On The Living Edge / COTLE 3-on-3
Tournament. As a participant, I agree to play
by the rules of the tournament, and I understand the floor
monitors reserve the right to eject players from the game and/or
property due to rowdiness or disrespectfulness without refund
of fees paid.
Team Name:
Entry fee is $50/team. 3 players per team required—up to 5 players allowed. This 3-on-3 tournament is a
double elimination tournament so you are guaranteed to play at least 2 games. Make check payable to Church On
The Living Edge or COTLE.
Entry Form must be completed and entry fee paid by June 8, 2012. No refunds will be given. Returned
checks will be assessed all bank fees.

Faith Christian University, 6000 East Colonial Drive, Orlando FL. Each game will be played on half-size
court. Refreshments will also be sold during the games.
