2016-2017 School Social Work Service Delivery Model
School Social Worker (SSW) duties have been divided into two major categories of service delivery:
1. Social/Emotional Assessment Services - Duties include Social and Developmental Histories, case
staffing & Individual Education Plan (IEP) meetings, manifestation hearings, supportive services i.e. Counseling as a Related Service (CARS), behavioral groups at identified schools,
training/professional development, consultation, home/school/community linkage to address
barriers to education, crisis intervention, and parent contact as appropriate - i.e. parent
communication, home visits, etc.
2. Attendance Compliance and Other Related Mandated Services - Duties include attendance
monitoring and analysis, attendance interventions through Problem Solving Team (PST) meetings,
IEP meetings, counseling (individual, small group, or large group at identified schools), virtual
instruction and home education follow-up, parent/student/teacher conferences, mentorship, case
management services, training/professional development, consultation, homelessness,
home/school/community linkage to address barriers to education, crisis intervention, and parent
contact as appropriate - i.e. parent communication, home visits, etc.
The District has been divided into East and West sections for service delivery by 14 social workers.
 Seven SSW’s have been assigned to Eastside schools
 Seven SSW’s have been assigned to Westside schools.
Two additional SSW’s have district-wide responsibilities:
o The Homeless Liaison is a 100% grant funded position; this person serves the entire
district.
o The Court Liaison for Attendance is responsible for State Attorney cases, Children in Need
of Services and Families in Need of Services (CINS/FINS), Teen Court, ASAP referrals,
and other supportive services as needed, i.e. Driver License, Learnfare, District Student
Placement Committee (DSPC), etc., for the entire district.
School-level Service Delivery Structure:
Two SSW’s have been assigned to each school; one will be responsible for social/emotional assessment
services and the other will address attendance compliance and other related mandated services. Each of
these individuals will be responsible for collaborating and working with the designated school-based
personnel (i.e. attendance clerks, attendance administrators, school counselors, PST chairs, IEP facilitators,
etc.).
 During scheduled team meetings, social workers will review referrals and determine
necessary plan of action for cases.
 Feedback will be sent to key team members at the schools on a regular basis.
Request for Services:
The designated SSW for each assigned cluster will receive and monitor their core school referrals. The
normal attendance and social history referral processes will be followed and reviewed with each school in
August 2016.
Attendance at PST, IEP, Functional Behavioral Assessment (FBA), and other meetings:
SSW’s will attend these meetings as needed and as their schedules permit.
Note: Referrals generated by private schools, charter schools, Child Find, Halifax Behavioral,
Hospital/Homebound, P.A.C.E., Volusia Virtual and Home Education will be assigned to the School Social Worker

EASTSIDE
There are a total of 36 schools on the Eastside. Four SSW’s will be responsible for social/emotional
assessment duties and three SSW’s will be responsible for attendance and other compliance issues.



Each of the four SSW’s assigned social/emotional duties will service 9 schools.
o SSW’s responsible for social assessments will have CARS schools* within their assigned
area.
Each of the three SSW’s assigned attendance related duties will service 12 schools.
The seven Eastside SSW’s will be located at EDC @ Atlantic, Portable 8B/Portable 5.

WESTSIDE
There are a total of 34 schools on the Westside. Four SSW’s will be responsible for social/emotional
assessment duties and three SSW’s will be responsible for attendance and other compliance issues.



Two of the four SSW’s assigned social/emotional related duties will service 9 schools each and two
will service 8 schools each.
o SSW’s responsible for social assessment duties will have CARS schools* within their
assigned area.
Three SSW’s have been assigned attendance related duties; one will service 12 schools and the
other two will service 11 schools each.
Six Westside SSW’s will be located at DeLand High School, portable 41 and one at DeLand
Middle School.
