Only 30% of Americans have their healthcare wishes documented. The
Muscatine Co. Advance Care Planning Initiative will connect you with a
trained facilitator who will help you create an advance care plan. Advance
Care Planning is a way for you to talk about your future healthcare goals.
You will make a written plan, a road map if you will, that will describe your
wishes to others if you cannot communicate.
This service is available at no cost.
Trained facilitators are available to help you create or update an Advance
Care Plan. This service is available for adults, age 18 and older.
