LIABILITY RELEASE:
In signing this release for myself and for any named immediate minor
(under 18 years of age) family member, I acknowledge & understand that
the participant will be exposed to risks of serious bodily injury, sickness or
death due to circumstances inherent in the COG facilities, activities and
programs, including the negligent acts or omissions of others. In exchange for being permitted to participate, I/we voluntarily agree to assume all risks inherent in participating in such activities, programs &
facilities. I acknowledge that I/we have no physical limitations or disabilities of any kind which would restrict participant from participating. I agree
to make City staff aware of any special accommodations that may be
needed at the time of registration and I agree to note such accommodation on said form. Despite these risks, I agree to hold harmless and release the City of Groveport, its boards & council, officers, officials, employees, volunteers and other representatives from all claims of liability or
legal responsibility for any damage or loss of any kind, including personal
property or death, property damage and economic loss, arising from
participant’s use. Participant agrees to follow all facility, activity or program rules, and realizes that his/her right to participate may be terminated by the City at any point in time for not adhering to said rules. I do
hereby grant and give these groups the right to use my/my child’s photograph or image, with or without my/my child’s name, both single and in
conjunction with other persons or objects for any and all purposes including, but not limited to private or public presentations, publicity & promotion relating hereto. The City reserves the right to rule on any matter not
specifically covered and to change policies as necessary. I understand
that any information provided may be subject to Ohio Public Records
Law.

Are you or a member of your family (age 14 and
over) interested in officiating youth soccer
games? Contact Amy Van Huffel (Recreation

Is my child too young for the division that
matches their birthday? The “ U” in each age
group means under. So everyone in U5/U6 is
under 5 & 6 years, etc. Your child’s birth year
determines their division. Players are permitted
to play up one age division, but they can’t play
down.
The Ohio South State Referee Committee
(OSSRC) will be offering an entry level soccer
officials training and certification on March 1, 2,
and 4. Participants must attend all 3 days of the
training to become certified.
Certified officials earn more money than noncertified officials, so don’t miss this training

PARENTS CODE OF ETHICS:
I hereby pledge to provide positive support, care & encouragement for
my child in youth sports by following this Code of Ethics. I will encourage good sportsmanship by demonstrating positive support for all players, coaches and officials at every game, practice or other youth sports
event. I will place the emotional and physical well-being of my child
ahead of any personal desire to win. I will insist that my child play in a
safe and healthy environment. I will provide support for coaches and
officials working with child to provide a positive, enjoyable experience
for all. I will demand a drug, alcohol and tobacco free sports environment for my child and agree to assist by refraining from their use at all
youth sports events. I will remember that the game is for children and
not for adults. I will do my very best to make youth sports fun for my
child. I will ask my child to treat other players, coaches, fans and officials with respect regardless of race, sex, creed or ability. I will promise
to help my child enjoy the youth sports experience within my personal
constraints by assisting with coaching, being a respectful fan, providing
transportation or whatever I am capable of doing. I will require that my
child’s coach be trained in the responsibilities of being a youth sports
coach and that the coach agrees to the Coaches’ Code of Ethics. I will
do everything within my power to assist all youth sports organizations to
implement & enforce standards in youth sports.

I haven’t been contacted by a coach yet. Is
my child on a team? Yes, your child is on a
team. If you haven’t been contacted by March
19, please contact the Groveport Recreation
Department at 614-836–1000 to get your team
information.
Can I request a team, specific coach, or other players to be on my team? No. The only
requests we honor are siblings in the same age
division.
Can I pick my practice days / times? No,
that decision is made by the volunteer coach. If
you would like to volunteer to coach, please indicate it on your registration form.
Are rules modified for each age division?
Yes. We follow the small-sided format recommendation from US Youth Soccer and the National Alliance for Youth Sports’ (NAYS) National Standards for Youth Sports to ensure each
division is age appropriate.
What is the emphasis of this league? This
recreational league emphasizes fun & skill development.
My signature below signifies that I have read & fully understand the
Liability Release & Parents Code of Ethics. Failure to sign will prevent
processing of this registration.

REGISTRATION:
January 2—March 1. Accepted in person at the
Groveport Recreation Center or online.
WHO:
Boys & girls between 4.5 - 14 years of age.
(Age determined as of December 31, 2017).
REQUIRED EQUIPMENT:
Soccer Ball, Shin Guards, Soccer Cleats/Tennis
Shoes, Black Shorts & Water Bottle
PLAYER FEES:
U5/U6 - U12 : $45 Resident / $55 Non-Resident
U15: $70 Resident / $85 Non-Resident
(includes t-shirt style game jersey, game socks, &

Divisions
U5/U6 DIVISION (born in 2011 or 2012)
3 vs 3 (split team format), no goalie; size 3 ball; 20 yd. x 25
yd. field; games are four 8-minute quarters; GRD attempts
to evenly divide teams by grade, gender, shirt size & experience in order to create balance; coaches officiate games.
U7/U8 DIVISION (born in 2009 or 2010)
4 vs 4, no goalie; size 3 ball; 30 yd. x 35 yd. field; games
are four 12-minute quarters; GRD attempts to evenly divide teams by grade, shirt size & experience in attempt to
create balance; soccer officials will be utilized. Genders
will be split when possible.
U9/U10 DIVISION (born in 2007 or 2008)
6 vs. 6 (including goalie); size 4 ball; 40 yd. x 60 yd. field;
games are two 25-minute halves; players are assessed
(not drafted) to identify and divide the skill level amongst
teams to try and achieve league balance; soccer officials
will be utilized. Separate boys & girls divisions when possible. Mandatory Player Assessment on Sunday, 3/5 at
1:30 PM @ Cruiser Park.
U11/U12 DIVISION (born in 2005 or 2006)
PRACTICES:
Tentatively begin in late March & are based on
coach availability. 1-2 practices per week.
GAMES:
U6—U12: Small-sided format recommended by US
Youth Soccer. 8 game season beginning April 1.
Games are played on Saturday mornings/afternoons
& Sunday afternoons at Cruiser Park.
U15: plays in the MSSA
OTHER:
 Teams are not linked to the previous season.
 Special requests for players to be placed with
other players/coaches will not be honored, with
the exception of sibling requests.
 Players may play up a division, but not down.

(not drafted) to identify and divide the skill level amongst
teams to try and achieve league balance; soccer officials
will be utilized. Separate boys & girls divisions when possible. Teams will play in the MSSA and travel throughout
greater Columbus area for games.
