Field Trip Request Form
Please complete this form. Once you have completed this form, submit it by e-mail or mail to the Oconee Heritage
Center. After we receive the form, a staff member will contact you to confirm the field trip. On the day BEFORE the
field trip, you must call between 9am-5pm to confirm the field trip with an OHC staff member.
REQUIRED FEE: $2 PER STUDENT. OHC will contact you to confirm a head count 14 days
before your scheduled visit. An invoice will be sent based on that head count.
