Job Purpose
Work under supervision to deliver a high quality, person-centred accountable
statutory social care service, ensuring that adults, their families and carers;
 are able to find their own solutions, assess, manage and review their own
needs, risks and capacity, maximise their own independence and self-direct
the responses to deliver agreed outcomes
 can access community opportunities, formalised personalised care and
support to deliver their expressed outcomes and maintain themselves in the
community
 are protected and safeguarded
Main Duties and Responsibilities

Ensure high quality responses to all adults eligible for social care services by;
a. providing appropriate responses or referrals within required
timescales, organisational, policy and legislative requirements
b. providing prompt support to meet immediate risks and needs in
crisis situations
c. providing targeted, appropriate support to adults in seeking their
own solutions, self-assessments of their needs, working with
colleagues in social care, health and other partner agencies,
contribute to multi-disciplinary assessments as required
d. ensuring that responses are geared to ethnic, religious, cultural, age
and linguistic background, practicing in a manner and delivering
services which are anti-discriminatory
e. that concerns and complaints are responded to and are addressed
by making appropriate changes to existing practices

staff, as necessary so as to enable adults to take risks whilst protecting them
from abuse, managing the risks to staff and the Council
Promote independence and facilitate solutions through self-directed
support, providing practical, person-centred assistance to adults, their
family/carer and their support network to:
a. assess and review their own risks and needs, seeking opportunities
for engagement in the community, asset based solutions and ,
where appropriate determine the allocated personal budget
b. identify community and other natural support resources, using asset
based approaches, maximising individual’s assets using,
preventative/universal services and other funding sources

c. To provide a high-performing social care/work service in
accordance with departmental policies and procedures and
statutory requirements.
d. design and cost a mutually-agreed best value support plan to meet
agreed outcomes, exploring all options including cash or managed
personal budgets
e. liaising and negotiating with Informal and formal support networks,
local providers and other resources as appropriate to deliver better
outcomes
f. access, use and review services, including the effective use of cash
payments and the safe use of equipment and adaptations in
compliance with relevant legislation, policy and guidelines
g. identify and implement positive solutions in resolving conflicts
h. arrange, implement and review support plans if requested by the
adult, and providing intensive support if they require it to start
activities or assess potential services
i. Seek to apply community, asset based solutions to support
individuals in their identified outcomes
j. Refer to the line manager unresolved support and service issues,
including
a)complex care and support arrangements
b)cases where it appears that outcomes to meet needs cannot be
delivered by the personal budget allocated
c) lack of availability of suitable services
d)local providers’ apparent failure to meet national or locally
determined quality or performance standards
Contribute to team effectiveness and response by working to Departmental
standards and, where joint working is in place, those of the NHS;

Provide assessment, care planning and review functions for adults who
have personal budgets and/or where services are commissioned by the
council, including review of services’ quality and achievement of outcomes.
Making necessary adjustments to those arrangements in line with the
changing needs of the individual, including wherever possible their
personalised needs and wishes
Undertake the statutory functions of the Department relevant to the post,
including legislation, policy and service developments in social care,
occupational therapy and related areas of skill and expertise
Maintain efficient and reliable liaison with other statutory and voluntary
agencies as well as other Council departments, having particular regard to
effective working relationships with statutory and independent sector
providers of residential, nursing and community based services
Develop expertise and knowledge of services, conditions and local
community resources and to use this to contribute to the management and
development of team, Group and council-wide policies, processes, projects
and services
Maintain an up to date knowledge of the statutory functions of the
department, relevant to the post, including legislation, policy and service
developments in social care, occupational therapy and related areas of
skills and experience

The nature of the work may involve the post-holder carrying out work
outside of normal working hours.
The post-holder may be required to attend, from time to time, training
courses, conferences, seminars or other meetings as required by his/her
own training needs and the needs of the service.
Expenses will be paid in accordance with the Local Conditions of Service.
This post is eligible for a DBS check under the Rehabilitation of Offenders Act 1974
(Exceptions) order 1975 (i.e. it involves certain activities in relation to children
and/or adults) and is defined as regulated activity under Part 1 of the Safeguarding
Vulnerable Groups Act 2006. Therefore a DBS enhanced check for a regulated
activity (includes a barred list check) is an essential requirement
