South London and Maudsley NHS Foundation Trust The Supported Living Team Information for service users Page Getting to know us We are a new community mental health team (CMHT) in Southwark. We have nurses, social workers, psychiatrists, psychologists, a benefits worker and an occupational worker. This leaflet explains what we do and how we work. If something's not clear - or there's something else you need to know - give us a call. What we do We can help you if you: • have difficult mental health and social problems • live in supported accommodation in the London Borough of Southwark. How can I get referred to the service? We take referrals from any source. They usually come from GPs, carers, relatives, staff in supported accommodation - or you can refer yourself. If you are referred, we will agree with you a convenient time to meet, either at your home or at our team base. When we first meet, a member of the team will ask questions about: • any current problems • how you are coping • your present circumstances • any things you are finding difficult • any help you already have • what help you might want - and need. This first visit will usually last about an hour. After this first visit, we discuss your situation to decide whether we are the right service for you, or if another might be better. Page 2 What we can offer you If we are the right service for you we will come back to talk over with you what we should plan to do. We might suggest: • talking with someone, like our team psychologist • looking at ways to get involved in local activities • talking with our psychiatrist about any medication you are taking or that you might find helpful. Once we have agreed a care plan with you we'll let your GP know. What else can we offer? • Psychological therapies (talking treatments). • Help to manage your medication better. • Advice and help with alcohol or drugs. • Support, information and advice to relatives and carers. • An interpreter, if you need one.
