assessment and notices a student has recorded their answers in the Question Booklet only, the teacher
may supervise the student transcribing their answers into the Answer Booklet, with no additional changes.

PDF and MP3 audio.

EQAO will be offering Braille in the following versions: Unified English Braille (UEB) and Nemeth. This is the
last year that Nemeth will be produced.

Remember…
✓ ALL school staff involved in the administration must read and adhere to the “Professional Responsibilities
for the Administration of the Grade 9 Assessment” in this guide.
✓ All provisions outlined in the administration and accommodation guides must be adhered to whether the
student is taking the assessment in a paper or an electronic format. No instructional material, including
electronic instructional applications, that facilitate responses to questions or that require internet
connectivity can be used during the assessment. We rely on the professional judgment of educators
to administer the assessments in accordance with EQAO guidelines and to ensure the security and
validity of the assessments.
✓ Students are not required to have an Individual Education Plan (IEP), or a special provision, to have the
questions read to them. Only students who request it should have the questions read to them. Readers
are not permitted to define, translate, explain or review any terms.
✓ All resources listed in this guide are available in the “Educators” section of EQAO’s Web site,
www.eqao.com, unless otherwise noted.
✓ There is a “Student Materials Order Checklist” in the Guide for Accommodations and Special Provisions.
Schools are invited to make copies of this checklist in order to identify special orders prior to entering
them into the Student Data Collection (SDC) system.
✓ Identify students who will write in an alternative location, and, at least six weeks in advance (if possible),
request permission from EQAO for them to do so.
✓ All students working toward a credit in Grade 9 applied or academic mathematics must write the
assessment. This includes adult students and English language learners.
✓ Assessment materials should not be copied in any way or for any reason.
✓ The use of cellphones, audio- or video-recording devices, digital music players or e-mail or text-messaging
devices during the assessment is prohibited.
✓ For ease of use, both Booklet 1s (Question and Answer) are presented as a single document for the audio
version, Braille version and assistive-technology formats (Word, accessible Word, PDF and accessible
PDF). This is also the case for both Booklet 2s (Question and Answer). Students will still receive the
printed booklets as two separate booklets. Although the layout of the printed booklets differs from the
layout of these versions, the content is the same. The assistive-technology formats will be available for
download from the secure site at 9:00 a.m., two days prior to the beginning of the administration period.
All responses will need to be stapled to the inside front cover of the appropriate Answer Booklet
and returned in the Special Versions envelope. All Question Booklets must also be returned, but they
should not be included in this envelope. Please note that although the page numbering may be different
between the PDF version and the printed version, the content is the same in both versions.

The Guide for Accommodations and Special Provisions is a companion document that provides information
about and outlines the required documentation for special provisions for English language learners and
accommodations for students with special education needs and special circumstances.
The Guide for Principals Preparing for the Administration of EQAO Tests is available on the secure section
of EQAO’s Web site (www.eqao.com) under “Student Data Collection System—Resources.”
Participation
All students working toward an academic or applied Grade 9 mathematics credit are required to participate.
All students eligible for the assessment, including English language learners, students with special education
needs and adult students, must be accounted for.
Do NOT include
■
students enrolled in a locally developed Grade 9 mathematics course or a mathematics credit recovery program
OR
■ students enrolled in a co-op course associated with Grade 9 mathematics who have previously passed the
Grade 9 academic or applied mathematics course.

Professional Responsibilities for the
Administration of the Grade 9 Assessment
All staff (including scribes and prompters) must read and adhere to the following.
Note: For more detail, please see the full document, also called “Professional Responsibilities for the
Administration of the Grade 9 Assessment of Mathematics,” posted on EQAO’s Web site (www.eqao.com).
Principals will ensure that
■
all eligible students (including those requiring accommodations or special provisions) are accounted for;
■
complete and accurate information on the students is provided to EQAO;
■
appropriate assessment materials have been ordered for all students;
■
all staff involved in the administration have been trained and have read this page;
■
scribes and prompters are aware of their roles and responsibilities;
■
all assessment materials have been received and are kept secure and confidential;
■
■
access to assessment materials occurs only on the morning of the day of administration, except in the case
of staff preparing materials for assistive technology and MP3 audio versions, who may have access to the
electronic versions two days before the assessment period begins; and
any suspected security breach related to the administration of the assessment is reported to EQAO immediately.
Teachers and principals will ensure that
■
no student or teacher assessment packages are opened prior to the start of the first administration session;
■
each student has sufficient writing room to work independently of every other student;
■
scribes and prompters have access to the assessment materials only during the administration;
■
assessment materials are not copied or removed from the assessment room during the administration;
■
all instructional materials related to mathematics in the assessment room are removed or covered;
■
students are supervised at all times during the administration;
■
■
Booklet 1 is completed in one continuous session, as is Booklet 2;
■
during the assessment, students do not receive
– instruction on any question from the assessment once the assessment materials have been opened;
– explanations, definitions, translation or examples of mathematics terminology or
– encouragement or influence to alter or revise their responses;
■
during and after the assessment, student booklets are not copied, darkened or corrected in any way;
■
■
no cellphones, audio- or video-recording devices, digital music players or e-mail or text-messaging devices are
used during the assessment;
the student booklets are not read or reviewed after the assessment is completed, unless they are being marked
in order to contribute to class marks; and
all circumstances that may affect the scoring of a student’s work are documented and reported to EQAO
on the Issues Envelope.
