Identify goals and aspirations and convert to service activities. Each participant has a service plan. Relationships established with participants and their families. Service information uploaded to database. Work with Engagement Officers to ensure plan is enabled. KRA 2: Monitor service provision against service plans Key Tasks Performance Measures Audit participants’ plans against service provision. Number of reviews completed. Services provided in line with plans. Review plans with participants and their families. KRA 3: Promote UCW Key Tasks Performance Measures Screen new referrals for suitability. Number of new referrals. Information sessions with community and family groups. Number of information sessions. Promote services to potential referrals. Promote service through LAC networks. Any other duties that may arise from time to time which fall within the parameters of this role and within the level of skills, competency and training of the incumbent. Section 4 Authority levels This position reports to the Manager Community Options and works within UCW’s Delegated Authority levels approved by the Board. Qualifications Current National Police Clearance Working With Children Check Current driver’s licence and competency in safe driving Knowledge Knowledge of the community sector, particularly mental health and disability services.
