Purpose of Post
This post is based within the Disability Team (0-25), to provide an effective
Occupational Therapy Service to children and young people with disabilities and their
families. To help people regain skills lost or learn new skills in managing and
functioning in their environment and to provide an effective person centred and coordinated assessment, care management and service provision to children and young
people with disabilities and their families and carers.
All staff will be expected to consider their role in the context of the objectives that the
Department is working towards and to contribute constructively to the continuous
improvement, performance management and best value culture and also the interagency context of the Department’s work.
Relationships
All staff will be expected to promote team working within their particular staff
group/service area but also across the Department as a whole, with corporate
colleagues, with staff from other agencies and representatives groups and working with
Elected Members as appropriate.

Main Duties and Responsibilities
1.
To carry out functional assessments of children and young people with
disabilities related to their daily living needs and to provide intervention to
support their development to live independently, comfortably with a sense of
dignity and autonomy.
2.
To adopt and implement a person centred approach/practice, which focuses on
the whole person and their family setting, addressing need and ensuring
consideration of aspirations, social inclusion and priorities of the family unit.
3.
To support and enable people in context to build on their own strengths,
expertise and networks to meet responsibilities, secure rights, and achieve
goals.
4.
To support and be instrumental in developing and maintaining a multidisciplinary service to the community of Hartlepool. To promote a culture of
collective responsibility and problem solving.
5.
To actively participate in inter-agency working, single assessment and practice
within the Care Management process to improve quality outcomes for people
who use the service and their carers.
6.
To provide case-work supervision and guidance to colleagues as appropriate to
the level of experience, competency and grade of Occupational Therapist.
7.
Provide information on services including access, eligibility, time scales and
costs as appropriate.
8.
Seek to ensure a quality service, which is effective, efficient and fairly
distributed.
9.
To ensure working practices and procedures in relation to care management
and service provision comply with Hartlepool Borough Council’s statutory
duties, functions, policy and procedural guidelines.
10.
To contribute to delivering objectives as identified in the business plan.
11.
To participate in the development of progressing the local and national agenda
of prevention and promotion of independence.
12.
To actively participate in agreed management process either individually or as
part of a team. (Supervision and appraisal, training and service development).
This will include Continuing Professional Development, which will be supported
by the department. Professional support to be provided through contact with
Principal Practitioner OTs in adults services.
13.
To work in accordance with the COT Code of Ethics for Occupational Therapy.
14.
Any other duties of a related nature which might reasonably be required and
allocated by the Disability Team (0-25) Team Manager.
Developments
The work of all Local Government departments changes and develops continuously
which in turn requires staff to adapt and adjust. The functions/responsibilities above
should not therefore be regarded as immutable but may change commensurate with
the grading of the post. Any such changes will naturally be the subject of discussion
and consultation.
