London Social Prescribing map
Background
As part of our constituent membership of Regional Voices’, LVSC has been mapping social prescribing initiatives in London since January 2015. LVSC
developed a Smart Survey for new and emerging social prescribers to complete to complete. Survey feedback has informed each social prescriber’s unique
page on the map.
Aims
The aim of the map is to gauge different prescribing models, how they operate, work in partnership and monitor & evaluate the impact of their initiatives. We
also want the map to serve as a resource / tool for:




Commissioners and policy makers to expand their knowledge about the range of services available, its added value and identify any potential gaps
with the view to roll out and commission social prescribing across London.
GPs in order to raise awareness of the range of services available, its added value and enable them to signpost patients and service users to non
clinical services.
Voluntary, community and social enterprise organisations (VCSE) to share information, expertise and good practice as well as enable
organisations to explore developing social prescribing projects in their areas.
Academia to find out about if and how prescribers are evaluated and how they may be able to support them in order to demonstrate their impact.
We have updated a number of other pages which can be read by clicking on the red dots on the map or on the pages below. Please note that the
red dots don’t reflect the actual geographical locations of the prescribers in each borough.
Unfortunately 2 projects have closed down due to lack of continued funding: HAGA (Haringey) and Prescribe Workshops (Southwark).
More projects have been added to the map including those in Merton, Westminster, Barnet, Camden, Greenwich, Islington and Kingston. We have
also included Green Gyms run by The Conservation Volunteers and volunteers.
We continue to work closely with the Healthy London Partnership to share and learn from good practice as well as roll out social prescribing across London.
The map
The map below shows the different prescribers in the boroughs they operate. Please note that some prescribers serve more than one borough. Others are
London-wide but at the present time serve a range of boroughs only (e.g. Prince’s Trust). All prescribers are indicated by a red dot. The name of the initiative
can be identified by moving the mouse over each dot. When you press Ctrl-left mouse, hover the finger over the red dot
until you see the hand appear and click – this will take you to the prescriber’s unique page. The whole document is
marked as final / read-only because we want to discourage users from editing as this may affect the dots and
hyperlinks on the map.
The last 3 pages also list all the projects with hyperlinks and the boroughs in which each social prescriber provides their
service(s).
