program offer in order to receive the most cost effective option which fits your

LOCAL PHARMACY OPTIONS:
Walgreens Prescription Savings Club
To join the Walgreens Rx Savings Club, go to any local Walgreens pharmacy and
store and ask at pharmacy to enroll in Rx saving club. Yearly fees are $20 for an
individual and $35 for a family. All individuals are eligible except if you are on a
federal funded plan such as Medicare or Medicaid.

Costco Pharmacies
To take advantage of any Costco Pharmacy, you DO NOT have to be a member of
Costco. If a representative asks you for your Costco Club Card when entering,
inform them you are only going to the pharmacy.
Just take your written prescription to the pharmacy and they will fill. To receive
pricing information before your prescription is filled, please contact your local
Costco pharmacy.

Rite Aid Pharmacy
NO annual or monthly fee.
Just sign up at a local Rite Aid Pharmacy.

Eastern Dental
Eastern Dental will perform EMERGENCY dental services for the negotiated fee
schedule rate under the Aetna Standard Dental Plan. If you require EMERGERNCY
dental service, Eastern Dental will accept as payment in full the rate that Aetna
Dental Standard plan would pay for that particular service. Please note this is for
