The Scleroderma Foundation Oregon Chapter
is currently selling raffle tickets for a
chance to win TWO ROUND TRIP TICKETS donated by Southwest Airlines to any destination
Southwest Airlines flies to in the Continental United States. Total value $800. Tickets expire 8/1/2016
Raffle tickets are $20 each.
For every ticket you buy, your name will be entered in the raffle, so
the more you buy, the more chances you have to win. Only 125 raffle tickets will be available for
purchase from now until Saturday, Sept.12, 2015 at 12:00 PM. Cash, check or credit card accepted.

By Mail Phone first for ticket availability. Purchase tickets by check made out to SDF – Oregon
Chapter, mail to PO Box 19296, Portland, OR 97280-0296. Checks in the mail must be received by
Saturday, Sept. 5, 2015 to be eligible for the drawing. Please send us the name, phone number and
email address that you want on the ticket(s). Upon receiving the check, we will fill out the ticket(s) with
your information, make a photo copy of it, tear the original ticket(s) off and put it in the drawing
container and return the photo copy along with the original ticket stub(s) for you to keep for your
record.
The winner will be selected Saturday, September 12, 2015

Winner need not be present in order to win.

Airline Tickets total value is $800.00. Tickets expire on 8/1/2016

Airline Tickets are subject to any restrictions and rules imposed by the airline including security fees,
security requirements, and reservations.

For raffle tickets purchased in person, the ticket-holder must ensure that his or her name, email
address and telephone number (including area code) are written on the portion of the raffle ticket that
is put in the drawing bin.

Odds of winning depend on the number of raffle tickets sold. (Maximum of 125 tickets will be sold.)

Winner must present a valid state issued photo ID or their ticket stub with the winning number to
receive the prize. If the winner is not present at the time of the drawing, they will be contacted by
phone to make arrangements for delivery of their airline tickets/voucher..

The Scleroderma Foundation - Oregon Chapter will be held harmless for any decision by the airline
that may affect travel, etc. The Scleroderma Foundation - Oregon Chapter is not responsible for
unreadable names or addresses, wrong phone numbers, or incorrect or unreadable email addresses.

