Lake County Community Health Improvement Partnership (CHIP) 700 South J Street Lakeview, OR 97630 8/19/2014 CAC/CHIP Senior Services Workgroup Meeting Minutes Date and Time: Place: 8/19/2014; 1:00pm – 2:30pm Lake District Hospital, Penn Wilbur South Conference room Partnership members, staff, and guests in attendance: Cecilia Montgomery, Rachel Klippenstein, Bernie Berkholder, Ken Kestner, Shree Morgan, Ray Sims, John Bunch, and John Adams I Call to Order 1:00pm – II Welcome and Introductions 1:05pm – III Business 1:10pm – John Adams provided a review of the agenda. Melinda Utley, Director of Long-Term Care at Lake Health District, provided an update on LHD’s plan to develop an assisted living facility. Long-term care will eventually become Lakeview Gardens, which will consist of 3 cottages, each with 12 rooms connected by central facilities. They will utilize the Greenhouse Project, which is a philosophy of care that helps seniors remain independent as long as possible in a home environment. There will be an opportunity to pursue a $1 million grant. The project is scheduled to break ground in spring of 2015, with 9 months of construction. The property just west of the hospital will be the site of the new facility. IV CHIP Strategies Update 1:20pm -John Adams provided updates related to the CHIP strategies. Lake County Mental Health received Transformation Grant funding for the Mental Health First Aid program for $38K, and they did not receive funding for the mental health integration with primary care. Additionally, the Physical Activity Scholarship program and the hospital’s Community Health Worker program all received Transformation Grant funding. -Physical Activity: John reported that the one pager is complete. The webpage address will be included on the physical activity promotional one pager. The webpage address is live; however, it is a work in progress. His plan is to disseminate the oral health and physical activity one pagers, together, as soon as the oral health one pager is developed. We received notice that our proposal to create a county-wide parks and recreation master plan with the Oregon Park and Recreation Department is a finalist to receive funding, and we’ll receive official word in mid-September. -Senior Services: The Senior Center was approved for the $20K grant for the Senior Navigator program Lake County CHIP Coordinator: John V. The ORH worked with John and the Senior Center to finalize the grant program language on August 1st. -Oral Health: CHIP will work with Advantage Dental to implement the fluoride varnish in the schools and the dental sealant program in the spring. Advantage Dental recommends pursuing an Oregon Community Foundation Grant for the dental sealant program and recommends that a community partner such as Lake Health District, the County, or the schools be the lead applicant to show strong community support. The LOI is due September. IV Strategic Planning 1:45pm – John reviewed the data and strategy prioritizations provided in the August 19th meeting minutes. The group agreed to support the data and strategy prioritizations. The group discussed the criteria to develop the strategies, including (1) the problem statement, (2) the strategy, (3) lead organization, (4) measurable outcomes and (5) budget. The group finalized the strategies including: Physical Activity for Seniors 1. The problem statement relates to the existing “There are insufficient services and resources for seniors within Lake County” problem statement. 2. The strategy is to create/organize needed physical activity programs for seniors, focusing on aerobic and muscle strengthening. Focus on low impact cardio/aerobic Provided in the Senior Center Program expanded to N. Lake when providers and facilities are available 3. Lake County CHIP and Senior Center to organize/lead the program. 4. The measurable outcome is to increase the number of seniors meeting the weekly recommended physical activity guideline of 150 min/week of aerobic activity (still need to figure out what percentage change?) 5. There is no budget related to this strategy. Connecting Seniors with Youth and Adults 1. The problem statement relates to the existing “There are insufficient services and resources for seniors within Lake County” problem statement. 2. The strategy is to connect seniors to youth programs and create a volunteer support group for the Senior Center. The Senior Navigator will help identify seniors interested in volunteering with youth programs, including FFA, 4H, and the Youth Mentorship program. Create a “Friends of the Lake County Seniors” organization to help provide Lake County CHIP Coordinator: John V. Adams Ofc. The Senior Center Board can lead the project. 4. Measurable outcomes include, increasing the number of seniors volunteering with youth programs, forming a new volunteer organization supporting Lake County seniors, and raising funds in support of Lake County seniors. 5. The budget is unidentified. Socializing Activities for Seniors 1. The problem statement relates to the existing “There are insufficient services and resources for seniors within Lake County” problem statement. 2. The strategy is to support socializing activities and programming for seniors in Lake County, including dances, hikes, trips, traveling, games, etc. 3. The Senior Center will be the lead for the project. 4. The measurable outcomes will include increasing the number of social activities for seniors and increasing the number of seniors involved in socializing activities. 5. The budget is unidentified. VII Next meeting/Adjourn 2:30pm – John discussed that the meeting is the final Senior Services workgroup meeting. The next CHIP/CAC meeting is scheduled for Thursday, October 23rd, 5:30-7pm, at the Lake District Hospital in the Penn Wilbur Conference Room.
