Especially for families living in Jefferson and Shelby Counties without medical insurance and with low income
(i.e.: qualify for Unemployment/Medicaid/Disability). Space is limited to the first 60 people on a first come,
first serve basis including a designated number of spots reserved for the Hispanic outreach ministry of the
church.
If you qualify and depending on outcome of vision services, bifocal glasses for adults are available for $75 and
single vision glasses for $60 to cover cost of lens materials. Referral services are available for children.

Directions to screening: Take I-65 South to Calera. Take exit 231 to merge onto US-31 S toward
Calera/Montevallo. Travel 2 miles and the building will be on the right hand side of the road. The sign in the front
says Christian Life Fellowship.
