Graduate assistants set up in payroll for fall semester
Must have U.S. based mailing address on file with UGA

Enrollment
• Automatic Enrollment Process
– Occurs late September for fall coverage
– Occurs late February for spring/summer coverage
• Complete the Enrollment Form to activate coverage
sooner!

Dependent Enrollment
• Can purchase coverage for eligible dependents

Waiver Requests
• Do you want to opt-out of the mandatory plan?
 Other coverage must meet certain minimum requirements
 Submit a waiver request each waiver period
Fall waivers apply to the period of 8/1 – 12/31
Spring/Summer waivers apply to the period of 1/1 – 7/31

• Waiver requests must be submitted by the
September 5, 2016 deadline – no exceptions!
Waiver Request – Random Audit
• Waiver requests may be randomly audited
during the semester.
– You will be contacted via your UGA email
– Must provide requested documentation about other health
insurance. Documentation must be in English.
– Charges for mandatory plan may be assessed if requested
information is not provided.
How Do I Pay Premiums?
• Student Account Charge
– Undergraduate international students
– Graduate international students who do not have a graduate
assistantship
– Doctor of Pharmacy students
• Payroll Deductions
– Graduate students with a qualified graduate assistantship,
fellowship or training grant
• Fellowship or training grant must be paid through UGA
payroll to be eligible

• Care outside of the Health Center (UHC)
• Must have a referral from the Health Center.
• Referral Exceptions:
• Medical emergency (follow-up treatment requires UHC
referral)
• Health Center is closed
• Treatment received more than 30 miles from campus
• Maternity, obstetrical and gynecological care
• Individual not eligible to use Health Center
• Use a Preferred Provider for services outside
of the Health Center
