This request is to be used by ASIC Registered Agents to lodge a Form 205A, 362, 484 or 6010, where:


an attempt to lodge online via ASIC's Agent Portal or EDGE software has failed, or
ASIC' s Agent Portal or EDGE software does not support the transaction you are attempting to lodge.
For ASIC to accept your document(s) you must:
- Be registered to use ASIC's Agent Portal or EDGE software
- Complete this request and email with your document(s) to [email protected].
- Complete the declaration and your Agent number
- Provide the company’s corporate key on Forms 484 and Form 362
