If you choose to edit the incident, the Problem

Module 20. Adding a disciplinary action to a Major or SESIR incident 1. After logging into your account go to the “Search” menu on the menu bar A. Please note that only data entry clerks and administrators will be allowed to enter administrative decisions for major and SESIR incidents. 2. Click on the “Pending Tier 1 Decision” sub‐menu A. Referrals which are pending an administrative decision will display with the most recent referral at the top of the list. 3. Click on the date of the referral that you wish to enter an administrative decision for. 4. A summary of the incident will be displayed as well as the section for entering the administrative decision. You can edit the incident before submitting the decision if needed. A. Please note: If you choose to edit the incident, the Problem Behavior will be cleared. All other information will remain. Please note the behavior prior to editing if this is not the field you wish to edit. Click “Edit” if you wish to revise the incident.
Select your disciplinary action(s) here. 5. If the summary for Part I and Part II of the incident is correct, scroll down to Part III to record the administrative decision(s). You may select as many as apply. A. Below the decision you may record additional comments. 6. Click “Submit” when you are done 7. An acknowledgment message will confirm that your decision has been recorded in the system. The referring staff will receive an email notifying them that the decision has been recorded.
