Thank you for taking on the important role of Team Captain!
By recruiting family, friends and co-workers to join your team and
support your efforts, you are giving hope to thousands of people
whose only chance at life is a transplant.
Over the years, our 5K Celebration of Life has gathered thousands
of participants, representing donor families, recipients, those
waiting, families who have lost loved ones while waiting,
volunteers, community groups and business and hospital
partners. We come together to honor those who gave, pay tribute
to those who received, offer hope to those who continue to wait
and remember the lives lost while waiting for the gift of life.
As NJ Sharing Network celebrates 30 years of saving lives, I
want to take this opportunity to thank you for your role in this
milestone. In just six years, the 5K Celebration of Life has raised
nearly $5 million and transformed the conversation on organ
and tissue donation in the state and across the country. In 2016,
we saved more lives than ever before by making 613 transplants
possible and your support helped make this a reality for those
recipients. You continue to inspire us each and every day.
This handbook is designed to give you tips and tools to make
team recruitment and fundraising fun and easy so that together
we can reach this year’s fundraising goal of $1.25 million!
Our staff is eager to help you, so please reach out to us at any

Our new and improved 5K website offers a fresh look for 2017 with the same user-friendly URL
and easier navigation across all three event fundraising websites. With a responsive design,
you’ll find the same information and essentially the same look whether you’re using your
desktop, tablet or smart phone!
WHAT’S NEW?
• We have upgraded our fundraising website and moved to a new platform called TeamRaiser.
• TeamRaiser offers a more intuitive and user-friendly sign-up process, while still allowing access to a
“personal headquarters.” Participants can customize their team or personal fundraising page, change their
fundraising goal, send emails to friends and family and even create a user-friendly URL for easy sharing.
• Since this is a brand new platform, all participants (new or returning) will need to create a new username
and password. Account usernames and passwords from previous years will not work!
IMPORTANT NOTES:
• The 5K Celebration of Life website includes three separate fundraising websites; one for each event
location. Total dollars raised are aggregated across all three fundraising websites to maintain one goal for
the 5K Celebration of Life. Top Fundraising Teams and Top Individual Fundraisers will also be aggregated
across all three fundraising websites. There will be not be a top team/fundraiser for each location.
• Registration fees are required for each event site and fees will apply.
• A list of all teams and team totals (regardless of event location) can be accessed by clicking on “View
All Teams” at the bottom of the 5K website.
• Making a contribution from the 5K website is easy! Click Contribute at the top of the page, and type in
the name of the participant or team to begin. This will search all three fundraising websites to find the
right participant or team.

SET UP YOUR TEAM & PERSONAL FUNDRAISING PAGE
Once you have signed up, set up your team and personal fundraising page. Personalize your page with photos
and videos, quotes, and your personal journey. If you haven’t already, be sure to send a copy of your team story
to [email protected] so we can display it on our story walls at the 5K Celebration of Life events.
REACH OUT & ASK FOR SUPPORT
Come up with a list of everyone in your personal network - family and friends, co-workers, doctors, and
even service providers. Decide on who you want to ask to join your team, and who you want to ask for
contributions in support of your team. Explain why you are seeking team members and contributions
and remember to be brief, but informative.
GET SOCIAL
Social media is perfect for fundraising and building your team. Post status updates
about the 5K with a link to your personal fundraising page, or create a Facebook
group for your team to communicate with each other. Post photos from last year’s
events on Instagram for all to see and keep a link to the 5K website in your bio!
UTILIZE TEAM CAPTAIN CENTRAL
Don’t forget to check out the Team Captain section of the 5K website. View important dates, past captain
correspondences and download fundraising tools and customizable materials to help you promote your
team. Email [email protected] to request materials such as green bracelets, posters or flyers!
STAND OUT! DESIGN A TEAM T-SHIRT
Proudly and creatively display your team name on a team t-shirt! Ask
your team members to wear it at team meetings and especially on event
day. Be creative with colors, pictures and messages. On event day, bring
an extra t-shirt with you and hang it on our Team T-Shirt Clothesline!
SHOW YOUR TEAM SPIRIT
Create a team sign or banner for team gatherings and event day. Post your selfies,
photos and videos of your team on our Facebook, Twitter and Instagram pages.
Use #NJSN5K and #CelebrateLife to show your support! You can also email
your photos to [email protected] and we’ll post them for you.
HOST A FUNDRAISING & AWARENESS EVENT
Many team captains and event participants coordinate fundraising and awareness events to benefit their
team. Examples include anniversary celebrations, sporting events, fundraising dinners and concerts,
jewelry sales events--the possibilities are endless! We will assist you when possible, as schedules and
timing permits, by providing materials, volunteer speakers and answering any questions you may have.
GET READY FOR EVENT DAY
Visit us for Early T-Shirt/Bib Pick Up to get your t-shirts and walk/race bibs ahead of time
(details on page 3). Come up with a team meeting place and time and be sure to let your
team know prior to event day. 5K Team Captain Conference Calls held prior to the event
provide important information and the opportunity to ask questions!

What is an employer matching gift?
Many companies set aside special funds each year to match contributions their employees make to qualified
non-profit organizations, such as the NJ Sharing Network Foundation. Most often, the companies match
contributions dollar for dollar, but matches can vary.
What is the Foundation’s Employer Identification Number (EIN)?
The NJ Sharing Network Foundation’s EIN is 20-2737719. Most companies will ask for this on their
matching gift forms.

TIPS FOR COLLECTING CHECK AND CASH CONTRIBUTIONS:
Individuals can easily make contributions online through the 5K website with a credit or debit card.
However, as team captain, you may quite often accept check and cash contributions from family, friends
and team members. These are considered “offline contributions” and will be entered on the website
once we receive them in our office. Due to heavy volume, please allow time for offline contributions to be
processed and visible on the 5K website. To ensure all contributions are credited to the correct participant
and team, please place each contribution in a separate envelope and include the following information:
1) Participant and team name (who should receive credit for this contribution?)
2) Name of contributor
3) Contribution amount and check number

innovative transplant research, family support, public awareness and education
about the life-saving benefits of organ and tissue donation and transplantation.
Clinical Advancement and Research
• A dvanced technical training for our clinical staff and educational programs for hospital staff.
• Three-year study in molecular immunology of clinical organ transplantation at our
state of the art laboratory.
Supporting Families
• Landscape of Life Wall & Meditation Garden honors organ and tissue donors, transplant recipients,
members of the community and also acknowledges contributions made in their honor or memory.
• Organ and tissue donor memorials at local hospitals.
• Wrapped in Love shawls, handmade by volunteers, as a tangible expression of comfort and compassion.
• Crisis funds available for new transplant recipients and those patients waiting.
• Scholarships awarded to high school seniors entering college whose lives have been impacted by organ
and tissue donation and/or are advocates.
Public Awareness and Education
• 5K Celebration of Life spreads awareness and supports families.
• Donate Life Float at the Tournament of Roses Parade honors donors and their families,
recipients and living donors.
• Team Liberty at the Transplant Games of America increases awareness and involves the
entire transplant community.

• Publication of a children’s book titled, Lily’s Little Life Lessons: Lily’s Laces, about a little girl
