COMMENTS

Food Service Establishment Plan Review requires an additional variable fee depending
on the size of the operation. If a variance is requested an additional fee may apply.
The fee covers normal application review, permit processing and two (2)
comprehensive, routine inspections per year. Formal complaints, a "call-backcompliance-inspection" or re-inspection other than the routine requires the payment
of an additional $100 if the inspection reveals an issue of non-compliance or other
critical issue.
*No charge after 1st call back if all violations are corrected completely.
*If some or all violations are not corrected completely within the specified time as
noted on the inspection report and/or new violations are noted, the establishment
shall pay a $50 Service-Administration fee per each violation not completely corrected
or for each new violation. Payment must be made to the Town of Southborough at
the Board of Health Office within 5 days of the infraction.
*If upon a third call-back inspection some or all of the violations have not been
corrected and/or new violations are noted, the establishment shall pay a $100
service/administration fee per each violation.
Failure to correct all violations before a 4th call-back could result in suspension or
possible revocation of the Food Establishment Permit.
*Failure to pay the Service/Administration Fee in the time required will result in
immediate suspension of the Food Establishment Permit until such time as the fee is
paid in full plus an additional $25 late filing fee.
